Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1226581297 2003-12-10 The tank battery consists of 1-100 bbl storage tank and a dike. There is no salt water tank and the separtor is located outside the dike.
-2085454322 2012-02-17 I conducted a status check on this site and found that the well was equipped with a plunger lift system and it was fenced with chain link. The tank battery consists of one 50 barrel stock tank equipped with flash arrestor and charcoal filter, and one separator. There was no brine storage tank present. The dike was free of vegetation and debris. The tank battery was fenced with chain link, and proper identification was posted. I found no leaks or spills at the tank battery or well head. No violations at this time.
282537268 2006-11-30 The Uniontown office received a call from Andrew Stanislav (330-629-6272 or 330-502-7451) at 7750 Amberwood Trail with a complaint about the tank battery and the lease road associated with this well. Mr. Stanislav lives directly south of the oil and gas facility. This inspection found that the tank battery has recently been relocated at the request of the surface owner to allow for new construction. Consequently, a new lease road and tank battery were built and the well and tank battery were fenced. There are no violations at this time. I called Mr. Stanislav and told him that the company was in compliance with DNR regulations.
86143573 2010-06-09 I performed a random field inspection. I GPS'd both the tank battery and the wellhead. I continued to walk around both the wellhead and the tank battery. This inspection satisfies 1501:9-9-05. There were no violations at this time.
-957089099 2007-03-20 This inspection was due to the Boardman Township Fire Departments request. The tank battery had an ID sign on it. The wellhead was south of tank battery. The well and tank battery were fenced in and locked. There were no violations at this time.
