Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1152210890 2006-07-06 The violations have been corrected. Compliance Notice
1299518004 2011-03-02 The latest bond posted by Maverick has been revoked. This well remains in production. The 16 day chart on the meter was dated 2/22/2011 and indicates production. The landowner is getting gas from the separator.
-153967008 2005-09-08 No activity has occurred to satisfy the outstanding NOV.
1558916597 2005-07-14 The Uniontown office received a call from Mr. Ed Draher (330-836-7850), the property owner at 1554 Earheart Drive reporting that an old abandoned 140 bbl tank was still inside the tank battery and that a pipeline trench across his property had not been properly backfilled. Murphy Oil cannot store abandoned equipment on site and is therefore required to remove the old tank or repair it and place it back into use. Secondly, Murphy Oil is required to backfill and restore the land surface around the pipeline trench.
-401652712 2009-02-19 The tank battery does not have the required identification. I did speak to Ed Dreyer, the landowner where the tank battery and well are located. He did not receive any notification from Maverick about the upcoming Commission hearing.
601893182 2004-03-18 Status check inspection - no violations at this time. The well has recently been transferred to Murphy Oil from G&O.
-992011831 2013-04-27 I inspected this well on 4/27/2013. The tank battery consisted a 210 Bbl. Stock tank, 25 Bbl. Plastic brine tank, a 2408 gas separator and gas orifice meter. The brine is completely full and the oil stock tank has approx. 15' of fluid. No oil / brine ratio is available. Last production was in 2009. All previous NOV's and Chief 's Order remain in place. The well consists of 4.5 casing with 1.5 tubing operated as a Plunger Lift well. Gauges on the casing indicate approx 300 psi. A domestic house service is presently being utilized by the landowner. Both the tank battery and wellhead are fenced and overgrown with vegetation. The top joint of the 1.5 tubing has a pinhole leak and will deteriotate without maintaince. The required I.D. placard needs updated information.
