1135795514 2005-12-28 Jack Fishburn called in to report oil spill that was being cleaned up by his trucks. Inspection of site and discussion with pumper showed that a valve in between the separator and tank was faulty which allowed approx. 1-2 bbls of oil to leak. Most of the oil was captured on site, some ran into road ditch and passed under County Road 24 through a culvert. A series of check dams were constructed on the east side of C.R. 24 and absorption pads were placed in the ditch. I advised pumper that additional pads were needed including down into the wooded area leading to the Alum Creek tributary. A small amount of oil scum was noted leading toward the stream but no oil or sheen was seen in the stream while on site.
1191522762 2007-10-04 On site to chk the report of a small oil spill. Apparently the flow line rusted out and allowed oil to come to the surface in a corn field. Clean up has been started and the farmer was contacted. He will pick the corn in the afected area to allow better clean up.
1206634997 2008-03-27 Approx 10;00 PM 3/26/08, Bob Atha phoned and notified me that oil has discharged from a field tile into a water way.

On site to chk the area with Bob Atha and Bob Wells. Oil has disscharged from a fild tile into a small water way in the woods to the north of the well head. The oil has traveled approx 200 yds to the north of the disscharge point and is approx 4 foot wide. An underflow dam will be built by the disscharge point and monitored for oil. Remaning oil will be removed form the surface of the water, oil soaked vegitation and sticks will be removed.

1207835764 2008-04-10 Chk on progress of clean up. Oil has washed out of a field tile and in to a wooded area. Some clean up done. As of this date, more oil has washed out of the tile. The absorbent boom in the water way has trapped most of it. A small amount has gone around/thru the boon and needs attention.

Phoned and talked to Bob Atha, advised him to: > Build an underflow dam in the area of the field tile out let. To capture any oil that may wash from the tile in the future. > Remove any residue that collects on the impoundment made by the dam. > Continue with the clean up of the water way and stick with it until the oil is removed.

-277007103 2012-11-14 Inspected the Bible #3 (#2587) as part of a standard productive well status check. The well is pumping by a green painted Alten electric powered pump jack. There is some oil staining around the well head and pad which they have mitigated with wood chips. The storage is shared with the Bible #1 and the well identification and emergency contact information is located at the storage. Status OK.
