| -1201454006 | 2013-01-09 | My inspection this day a request from landowner to verify status of well. I found tank with no identification, with no dike and not tied to well. The well has a pumping unit and while it IS tied to a separator, the valve is shut between the well and separator. There is a large amount of 1 tubing laying on the ground and a smaller pile of 2 tubing around the well. The operator has reported 9 bbl oil sold in 2011 and 4 bbl in 2010 - I will question how that is possible and believe an error has been made in reporting - no amount that small can be marketed. The well was transferred finally in 2012 after sold to the current operator in 2008. There is no record of assignment or bill of sale to Big M Petroleum and there was a Chief's Order #2006-17 that was not enforced but is in Constitution's name. Well needs produced or plugged and complete identification posted. | ||||

Told her to call Andy and have him do an inspection. Her attorney keeps telling her to call us to get a C. O. for the violation. I have explained that we have done all we can do. Gave her several options for what she could do next. | ||||

They had talked to Brian Morris (Big M) and believe he will take it - has service rig lined up to work on it - just needs to finalize the deal. I told them this has drug out too long - needed to get something done - well going - by end of summer. They agreed. | ||||
| 1906742067 | 2014-01-29 | Well is equipped for production with a pumping unit and a gas engine. Well has not been pumped for a long time. There is oxidation on moving parts of the pumping unit and the bridle is almost rusted through in several spots. There is a separator and a 100 barrel tank on site. There is no containment dike surrounding them. The 100 barrel tank has a leaky valve and nipple. I observed a drip during the inspection. Production records state that 14 mcf of gas was produced in 2012 which is not enough to meet commercial production standards as defined by ORC 1509.062. I attempted to call the company but the number has been disconnected. I will find a way to contact them and notify them of the ongoing issues with this well. | ||||
