1309358299 2011-06-29 Carl Roberts received call from Don Brown of 15285 W Pine Lake Rd, Salem ,OH on 06/28/2011. Relayed message to me. I called Mr. Brown (330-537-4079) on 06/29/2011 at 07:56. Mr. Brown is concerned that the well in question is not being produced. I conducted field inspection of well head and tank battery. No signs of leaks or spills, ID sign on stock tank, tank rusted and paint peeling, no brine tank, no separator, disconnected lines near meter, very grown up with vegetation. Well head showed no signs of production, no pumping unit, no rabbit catcher, top of tubing open to atmosphere. Very grown up with vegetation, well head within multiflora rose shrub, well head covered with wasp/hornet nests. No signs of service rig access. No access road. Called Danny Long & Sons (330-866-3558) on 06/29/2011 at 11:47. Did not make contact with operator. 06/30/2011 Received call from Danny Long (330-495-3429). Questioned him on production for said well. He agreed well was in poor condition. I informed Mr. Long that he needs to plug or produce well. Mr. Long wants to attempt to swab well one more time, within next two weeks. If no production then he agreed to plug well. I requested notification of swab date so I can witness.
1359477750 2013-01-09 I was on location to plug the above well, Nicks Well Pluggers were moving on location to plug the above well. Due to the ground conditions ( mud ) they will not start the plug job until tomorrow 1/10/13.
1373564032 2013-06-28 My inspection showed the presence of a battery tank and line adjacent to the former well site. I consulted with John Fleming who informed me that the tank (permit no. 453 in photo) was used to swab test the well being plugged (20424). He confirmed that he spoke with Dean Long who is in the process of removing the tank and line remaining onsite. The lease road and former well pad have been restored/graded and vegetation has returned. Final restoration failed pending removal of tank and line by operator.
2106315101 2011-07-22 Received notification from Danny Long (330-495-3429) of intent to swab at 10:00 on 07/22/2011. Met Danny Long and Don Brown on site at 09:30 on 07/22/2011. Stock tank placed in tree line next to well head. Original flow line to tank battery not operational, hole in line. Danny Long stated that all 2-3/8 tubing was removed from well, and a Jetstar tool was hung up in the 4-1/2 casing, around 5392'. Appalachian Energy on site to swab well at 10:20 on 07/22/2011. Appalachian hit fluid at depth of 2240', ran into tight spot at 4197'. Swabbed approximately 33 bbl of oil/brine from well. Danny Long reported that Appalachian will be back before 07/29/2011 to fish Jetstar tool from well and continue swabbing. Compliance notice NOT satisfied at this time.
440815167 2013-01-10 Nicks Well Pluggers was on location to plug the above well. All plugs were set, circulation was established, was topped off and cement stayed at the top. Plugging completed. The plugging report is on file.
-704335810 2011-08-12 Appalachian Energy on site to fish Jetstar tool from well and make final attempt at swabbing. Ran into tight spot in 4.5 casing at 4190' with fishing tool. Unsuccessful at fishing tool from well, unable to continue swabbing. Danny Long finally conceded to plugging well. This case continues. Compliance agreement not satisfied at this time.
