-1005337726 2014-11-26 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found no change from my inspection on Friday, 11/21/2014. I called and spoke with Steve Svetlak. Mr. Svetlak stated 8 loads of impacted soil and weeds mixed with mulch were removed from the site. I instructed Mr. Svetlak that: Phone Call
-1047154238 2014-11-06 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found the gas leak at the wellhead repaired and no longer bubbling. The rest of the clean-up appeared in the same condition as my inspection on 11/3/2014. Additional photos were taken. Follow-up inspections will continue.
-1059347449 2014-10-17 I was on location for a status check. During my inspection, I found the tank battery was completely engulfed in phragmite weeds. The tank battery consisted of a 100 barrel production tank and a 50 barrel brine tank. I was unable to inspect the dike. There was no identification sign posted. There was a separator located 50 feet from the tank battery and engulfed by weeds. I noticed that the flow line valve leading to the separator was closed. Next, I found an oil spill at the well. Oil covered the ground in a 40 foot by 30 foot area with an approximate depth of 3 inches. Bubbles were noticed next to the well with an unknown source. The oil spill resided on the north and west side of the well. The oil spill was estimated at 44 barrels. Upon further inspection, I found the well was not in production. The well was equipped with a plunger lift assembly. The flow line valve at the well was in the closed position. There was a domestic gas line coming off of the 4.5
1414069455 2014-10-18 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found Mr. Svetlak and Steve Stewart on location cleaning up the oil spill. Mr. Stewart was working a clearing phragmite weeds in and around the tank battery using a Bobcat Track Hoe. Upon further inspection, I found the 100 barrel production tank and 50 barrel brine tank in good condition. Both tanks had bull plugs on the load lines. There was a dike surrounding the tank battery. I noticed a sump pit had been dug near the well and Mr. Svetlak stated the oil and water would be pumped into the 100 barrel production tank. Also, Mr. Svetlak stated he was working on obtaining sawdust and peat moss to bio remediates the soil. Follow-up inspections will continue.
1414154495 2014-10-23 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found Mr. Svetlak on location cleaning and pumping the remaining oil blobs and water into the 50 barrel brine tank. Mr. Svetlak had the fitting and pipe to plumb the 8.625

During my inspection, I found the well site in the same condition as my previous inspection on Dec. 2, 2014. Additional photos were taken. I called and left a message with Mr. Svetlak requesting a return phone call. Follow-up inspections will continue.
1708835960 2015-02-04 I was on location for a follow-up in reference to compliance notice no. 1059347449. During my inspection, I found the fluid level in the 100 barrel production tank approximately two (2) feet from the top. The 50 barrel brine tank hatch was frozen shut. All of the impacted material was still on location and covered with snow. According to Kurt Kollar of the Ohio EPA, Mr. Svetlak was given until 2/28/2015 to completely remove the impacted material from location.
1984277093 2014-11-03 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found the condition of the oil spill clean-up the same as my inspection on 10/31/2014. The gas leak at the well still had not been repaired. I called Mr. Svetlak. Mr. Svetlak stated the clean-up would continue and that he was waiting on the soil sample results in order to dispose of the material properly. I instructed Mr. Svetlak to repair the gas leak immediately. Additional photos were taken. Follow-up inspections would continue.
2097305110 2014-11-21 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found the ball valve off of the 8.625
2135559065 2014-11-06 I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a plunger lift unit. A domestic gas line came off of the 4.5 casing. The tank battery consisted of a 100 barrel production tank and a separator. There was a dike around the tank. An identification sign was posted. The lease road was in stable condition with a locked gate. The well was placed into production prior to House Bill 278.
-337867321 2015-02-25 I was on location for a follow-up inspection in reference to compliance notice #1059347449. During my inspection, I found a trailer positioned near the rental house containing impacted material. The trailer was lined with plastic. A snow, covered path had been plowed leading to the tank battery and the remaining impacted material. The 100 barrel production tank was still full of oil and water. The hatch on the 50 barrel brine tank was frozen shut. The well was shut-in with the flow line ball valve in the closed position. All of the small, plastic containers except one had been shut-in. According to Kurt Kollar, Ohio EPA, Mr. Svetlak has until 2/28/2015 to remove the rest of the impacted material. A follow-up inspection will be conducted by 3/2/2015.
343324011 2015-03-02 I was on location for a follow-up inspection in reference to compliance notice no. 1059347449. During my inspection, I found Mr. Svetlak on location removing the remaining impacted material. Mr. Svetlak was using a bobcat to load the material into a lined trailer. Mr. Svetlak stated the material was going to the Carbon Limestone Landfill in Mahoning County. Mr. Svetlak estimated three (3) trailer loads of material were left on site. I reminded Mr. Svetlak to file annual production records for the well. A follow-up inspection will be conducted by 3/16/2015.
-475847793 2014-12-02 I was on location for a follow-up inspection in reference to Compliance Notice No. -1059347449. During my inspection, I found no additional removal of the impacted soil and weeds. The production tank and brine tank were completely full of water. The two containers with impacted soil and weeds were still on location. I called and left a message with Mr. Svetlak to return my call for an update on the oil spill clean-up operations. Follow-up inspections will continue.

During my inspection, I found the majority of the oil had been removed from the site. The impacted soil and weeds were piled and staged at the north end of the oil spill.
