1104250365	2004-12-16		Received complaint from landowner Joe Huff concerning production of wells on property that he purchased. There are 5 wells that are leased to Chris Neiset. The wells are not on the State of Ohio well database and are not on the list of bonded wells for Neiset. A notice of Violation will be issued to have the 5 wells added to the bond list for Neiset and to display the proper ID sign for each well. Since the API well number on this report is only a temporary number, the operator will be notified of the permanent API well number to be used on the ID sign for each well. The ID number will be assigned from the Columbus office. This well is located adjacent to the large storage tank on site. There is a pump on the well. Chris Neiset indicted that the well is being produced. There was a light snow cover on the ground. It did not appear as though there was any excessive oil on the ground other than that generally seen around a wellhead. Another check of the site will be done when ground conditions are clear. 1208202764	2008-04-14		Received complaint from landowner Joseph Huff that some wells on property are not being produced. This well has pumpjack and appears capable of production. ID sign is faded and needs to be replaced. Well is located next to tank and stone access road.
1327674005	2012-01-09		At the time of my inspection only improper identification was noted. This well is producing into a common tank with four other wells on the lease and well needs identified at tank and at well. Spoke with operator about proper identification.
2002524279	2011-07-19		8, 6 w/ casing head, tubing, rods, and pumpjack. Plastic production lines to tank. Small amount of oil around wellhead leaking from stuffing box and recent servicing of well. No ID posted. Talked w/ operator who will post ID and clean site.
