INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1404228757 2014-06-30 On the date of inspection I accompanied ODNR Inspector Randy Akins to witness the cementing of an intermediate casing string. After arriving on site we were informed that the cementing event would not occur until much later than was expected. Randy Akins and I left the site. I did not witness the cementing of the intermediate string. Refer to Randy Akins's report for further details.
1410801886 2014-09-15 I was advised that this well has 700 PSI on the annulus of the 9-5/8 inch diameter casing string. I then met and talked to Hess Ohio Company man and Drilling Operations Supervisors Bob Mcspadden and John Mode. We went over the cementing and also the fact that this well is one on several on the same pad which has pressure on the annulus. I advised that a remedial plan would need sent in for approval before this well could be placed into production.
1418831076 2014-12-12 I am on site today to check on the range of chlorides being captured in the top left hand corner of the drill pad. I found that fluid from the pad is still going into the sump. The reading today are at 480 PPM and is dropping from 800 PPM. H & P Drilling will be running production casing later tonight.
