-1688719761 2007-11-16 The Hocking County 911 contacted the ODNR, Division of Mineral Resources Management, at 6:10pm on 11/16/2007, regarding an oil spill from this well. An inspection conducted on 11/16/2007 revealed that the source of the leak was from an open saltwater valve and dike drain that had been removed. Oil and brine was then released down a driveway and entered alongside Jake Tom Road. A low lying section near the road (approx 200 feet from tank battery) was found to contain contamination and pollution. The size of the spill at this location was 31'x 9'. The contaminant then entered a frech-drain system, and was discharged into a field directly across the road. This size of the spill at this location was 7' x 18'. Test holes had been dug, in order to determine how far contaminant traveled, before ODNR arrived on-site. The Hocking County ER crew was directed to contain the spill. Absorbent pads and booms were utilized. The well owner, Paul Nutter had been contacted earlier by the Hocking County ER crew and stated that he sold this well to Steve Vandergrift. Mr. Nutter also stated that he would however, still provide assistance with the spill. Mr. Vandergrift arrived on-site and was informed of the situation. Mr. Vandergrift stated that he would begin cleanup immediately. Mr. Vandergrift states that he checked on this well around 3:30pm today and there was no problem. He was also provided guidance on spill cleanup and containment. The spill was contained and a crew was then scheduled to perform required work on 11/17/2007.

-481918428 2007-01-24 An inspection conducted on 1/24/2007, revealed that the dike was not capable of preventing the escape of brine. A valve will be required for this dike to prevent discharge of brine and oil. It was noted that a pipe was routed from inside the dike to the edge of the lease road.
-647072348 2012-03-07 Received complaint via telephone from EPA employee Trevor Irwin at 9:10 am alleging water running down a lease road, then along the county road in a ditch line at 13869 Jack Tom Rd., Logan, OH. I arrived at the site at 10:49 am. Where the water had flowed was visibly evident due to the formation of salt crystals on the surface. Residual water in the dike, and two wet spots along the county road were tested with high range Quanta's. The water in the dike tested at 7.2 units which translates to 2790 parts per million (milligrams per Liter) chlorides. The two wet spots that were tested maxed the high range Quanta's out which means chloride concentrations were above 6765 parts per million (milligrams per Liter) chlorides. Mr. Steve Vandagriff was called at 10:54 am and a message was left on his home phone and his cell phone. He called back at 12:35 pm and we set up a time to meet at the site. Once at the site I informed him that the issue was complaint driven and we discussed remedial action. He admitted to draining the water out of the dike under the impression that it was fresh water.
-756002914 2012-03-08 Inspected work in progress on the clean-up of the brine spill. Areas had been worked with an excavator. The use of lime was evident. They also spread straw on the surface.

955769732 2007-11-17 A follow-up inspection revealed that Pineco has begun cleaning up the impacted areas. Peat moss & Gator was applied alongside the road and to impacted areas located off the road. Quantab showed 6.4ppm in area containing 7'x18' spill. The company was informed to remove and provide new topsoil, apply lime, seed & straw.
