1379002915 2013-09-11 This inspection was conducted as a NOV follow-up to NOV# 678689782. Jeff Fry accompanied me on this inspection. The pit has been closed. No contamination was observed during the inspection in or around the pit area. The violation has been resolved.
1380137126 2013-09-20 This inspection was conducted as a NOV follow-up to NOV #1692506664. The inspection revealed that the rig and drilling equipment has been removed. The area around the well has been graded. No seed or straw. Identification has been posted on the pumping unit. Violations have been resolved. Preliminary restoration will be passed when vegetation/erosion control is established.
1480454957 2012-07-06 I inspected this location today. No one was working during inspection. The pit was not closed and soil has not been removed.
1692506664 2013-09-11 This inspection was conducted as a preliminary restoration status check. Jeff Fry accompanied me on this visit. The inspection revealed that the well is equipped with a pumping unit and motor. It was not pumping during inspection. The rig that was used to drill the well was rigged up on the well and other various parts of the rig were laying on the location. The area around the well has not been graded with a dozer. No identification was posted at the wellhead. According to ORC 1509.072 the owner is required to remove all drilling supplies and drilling equipment within two months after the date upon which the drilling of a well is completed. Also, as part of ORC 1509.072, the owner is required to grade or terrace and plant, seed, or sod the area disturbed that is not required in production of the well where necessary to bind the soil and prevent substantial erosion and sedimentation. Proper identification with the owner, permit number, well number, lease name, county, and an emergency phone number is required at the tank battery and each wellhead as multiple wells are being produced into a common tank battery. The owner will need to resolve these violations by 9/20/2013. Failure to comply may result in further enforcement action. Photos were taken for documentation purposes.
-1867894267 2012-06-29 I made a phone call to Mr. Randy Moran concerning the status of the pit closure. I extended the deadline for the pit to be closed and soil to be removed to 7/6/2012. I informed Randy that if the deadline is not met the contaminated soil will have to be hauled to an Ohio EPA approved landfill.

The mud pit for the well has oil in it. The liner is not containing the oil in the pit. The fluid in the pit will need pumped out and the liner removed. Any soil that came in contact with the oil will need to be removed and taken off site. Inspection revealed there was no permit posted anywhere at the rig.

789747561 2012-06-21 I made a phone call to Mr. Randy Moran concerning the pit liner not containing oil in the pit. He informed me that Dean Moran still needs to clean out well and will need the pit. I informed Randy that soil will still need to be removed when finished with the pit.
927338913 2012-01-23 An inspection of drilling operations revealed no violations. The depth at time of inspection was 560' and approximately 16' from top of Berea.
