1213204238 2008-06-11 I witnessed the cementing of the 8 5/8 casing. I found no violations at this time.
1213361417 2008-06-12 I performed a rig inspection. The fence was down around some of the location. I called Mr. Nick Paparodis, 330-565-6940 and let him know that the fence needs to be put back up on the posts around the location. He would do that. The drilling depth was 1741'.
1213644257 2008-06-16 I performed a preliminary restoration status check. The drilling pit fluid was 3 foot down. A fence was around the drilling pits. The rig equipment was still on the location. The ID sign was posted. This well does not pass preliminary restoration.
1234987430 2009-02-18 I performed a preliminary restoration status check. The drilling rig is gone and the debris was removed. The drilling pits were solidified and closed. The privacy fence with barbed wire is up as will as the ID signs. The well is in the flea market's parking lot. This well has PASSED PRELIMINARY RESTORATION.
-1388781483 2008-10-20 I performed a preliminary restoration status check. The drilling pits are closed. The wellhead is open. D & L Energy have not frac'd the well yet. The tank battery has not been started. This well is in the middle of a large flea market parking lot. I called Dave Rice from D & L. He stated that they are going to try and frac the well in 3 to 4 weeks. This well has not passed preliminary restoration.
1618071963 2009-05-11 I performed an inspection on the location and the surrounding area. The tree and stump were hauled away. This complaint is resolved.
-1712860809 2008-06-11 I performed a rig inspection. I GPS'd the well. The pits and the pit liners were in good shape as well as the location. The fence was around the pits and the location. The ID sign was posted with the permit. There were no violations at this time.
-1725650840 2009-05-05 I received a call from Mr. Dennis Barr(330-755-6683) about tubing and thread protectors still on the location. He uses the parking lot for his flea market. He met him on the location along with Dave Jenkins(330-506-6509) of Hard Rock Excavating. Mr. Barr was very irritated about the stuff left by the tank battery and thread protectors left around the wellhead. Mr. Barr was yelling a lot. I tried 3 times to calm Mr. Barr down. He kept the yelling up. I finally told him that I have had enough. He stated that it is your call. Upon that, I left him and talked to Dave Jenkins about getting some of the stuff picked up. The thread protectors, tubing, were hauled off. They brought one load of rock for the location. They will bring 5 more loads tomorrow. They will also haul a small tree that they took down while installing the production. Line. This case continues.
2103460975 2008-12-18 I performed a preliminary restoration status check. The service rig(Whipstock) was on the well. They had swabbed the well in and ready to run into well with tubing. The drilling rig and the debris are gone. The drilling pits are closed. This well has not passed preliminary restoration.
2111902726 2009-01-20 I performed a preliminary restoration status check. I GPS'd the new tank battery site and the wellhead. The drilling pits were closed and the debris hauled off. The ID sign was posted. The area around the wellhead is a parking lot. No fence has been installed yet around both the tank battery and the wellhead. This well has not passed preliminary restoration.
231607706 2008-06-25 I performed a preliminary restoration status check. The drilling pit fluid had been vacuumed out but 2'. Some of the fence was down. I called Mr. Nick Paparodis, D&L Energy land VP. I stated that the some of the fence around the pits were down and they needed to be back up by the end of today. He said he would take care of it. This well has not passed preliminary restoration.
255527448 2008-07-17 The fence was around both locations. I witnessed the solidifying and the closing of one of the drilling pits. I will come back in the morning to check the other.
-338475926 2009-05-08 I performed a complaint check up. The tubing, thread protectors are gone from around the tank battery and the wellhead. A total of 12 extra loads of rock were hauled into the drilling location and spread out. Rock was also spread ontop of the production line to the east of the wellhead. Dave Jenkins from Hard Rock Excavating was there taking pictures for D & L Energy. Dave stated that the tree should be out by the end of today. The tree was taken out during the installing of the production line. Not the drilling. I told him that I would check the location Monday. This case continues.
574325349 2008-09-10 I performed a preliminary restoration status check. The well was GPS'd. The pits are closed and the top soil spread out on top of the parking lot. The well had not been frac'd yet. The frac tanks were on location. This well has not passed preliminary restoration.
738537835 2009-02-03 I performed a preliminary restoration inspection. I GPS'd both the wellhead and the tank battery. The pits are closed and the debris is gone. The drilling rig is gone. The tank battery has been built. The wellhead is in a Flea marked parking lot. The ID signs are posted. The fences are up. But the privacy fence has not been installed. This well has not passed preliminary restoration.
-889836835 2008-07-14 Jay Cheslock and I performed a preliminary restoration inspection. The fence around the drilling pits was down on the northwest and west side of the pits. I had called D & L Energy repeatly about the fence being down. They put the fence back up. This flea market parking lot has cars and people in the area during the week and weekend. The pits need to be closed. Close the pits.
-965585351 2008-07-18 I performed a NOV follow-up. Both the drilling pits were solidified and closed. The debris was removed. The drilling equipment is gone. The rest of the top soil was being hauled to the tank battery area. The parking lot of the flea market is put back enough for cars to park. The NOV is resolved. The preliminary restoration has not been passed.
