Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1052429706 2014-04-24 My inspection finds the service rig pulled three joints of 2 tubing. The rig crew then ran the wireline tools to a depth of 1510'. The rig crew then began mixing prepared clay and washing it in. I instructed the rig crew to check the top of the plug the following morning. Compliance Notice
-1120266334 2007-11-02 Talked to operator
1398723189 2014-04-28 Witnessed clay being washed to the surface. It was tagged when the crew started at 178 ft. Spoke with well owner Mr. James Williams and rig operator Mr. Darren Ellis.
-1488041654 2014-09-29 Upon inspection, it was observed that the former well location has been graded and is stabilized with a full vegetative cover. The lease road is a township road and will not need to be reclaimed. All production equipment has been removed from the location. No pit was used as this was a clay plug job. This fulfills the requirements of the final restoration.
-1634063078 2007-05-29 Re-inspect this day found no change, well still not producing. Jack is on well, enclosed with small building, well has good ID's - no permit no but I am having trouble determining which well belongs to which number as are not plotting close to location on map. Much leaf cover and in hollow so difficult to get good GPS reading
1939750673 2008-04-18 Inspection this day found well producing with jack, ground cleaned up, tanks have been centralized and hauled out, ID's at every well - even tin and excess old steel line running in creeks is being picked up and sold for salvage. Lease looked much better, NOV resolved.
-1940403132 2014-05-14 Inspected well to see where the clay level was after it was plugged. It was at the surface. Accompanied by Mr. James Williams.
-1956448471 2007-10-31 Inspection found rig on location
282444781 2010-09-02 Inspection this day found well with jack, rig has already been on this well and is producing. Good ID's at well, tanks are in process of being moved.
885979937 2004-12-28 Inspection this day found well with jack, has no motor, no belts and not capable of producing, tied into line, tho. Several tanks out in the woods with no ID's so not sure where it's going. ID's say Heinrich owns, at well, needs added to his bond. Location plots a little far away from well, but ID's at well say Pugh so believe this is correct. 5/25/2007 Re-inspect this day. No change, has come in and emptied tanks. 11/1/2007 New owner requests additional time. He has serviced the well but still won't pump and needs to get a different rig on well, which he says he probably can't do until winter passes. Will get it producing as soon as possible.
