1307559723 2011-06-08 Meet Trevor Irwin with OEPA. Follow up clean up of oil spill from lighting strike at storage tank. Called Mark Fluehart went over placing a dam above catch basin. Meet with landowner went over clean up process. Approximately 90 % of contaminated oil and soil has been stock piled for removal from site. Took pictures of site clean up.
1307716634 2011-06-10 Meet with Mark fluhart at site. Oil and soil has been removed from site, all areas that have been affected are seeded and mulched. Took follow up pictures.
1492104845 2011-06-07 Call from Bill with Northwood energy @ 12:30 pm. A lighting strike hit a 210 bl. Storage tank located along twp. Rd. 212 1 mile north west of st. 643. 3 Rivers fire department on scene. Pumper said there was around 150 bl. Lost from tank rupture. Approximately 100 bl. Recovered by vac truck inside dike. A estimated 30 bl. Was burnt and approximately 20 bl. ran north approximately 100' along twp. Rd. 212 then crossed thru a culvert to the east and continued 200' to a unnamed water way. Oil then ran approximately a 1/4 mile to the south east and was contained. Clean up was started day of spill by Mark Fluehart who is a contract pumper for company. Called and notified OEPA this date.

2087287609 2013-12-11 On the day of my inspection I found that the well head was equipped with a pumping unit. The tank battery consisted of one 210 barrel stock tank, one separator and an earthen dike. Proper identification was posted and there were no leaks or spills. No violations at this time.
