112108651 2012-08-09 On 8/9/2012, I conducted an inspection of the newly drilled well. Appalachian Well Survey had perforated the well with a top shot at 5,180' and bottom shot at 5,338'.
1345811370 2012-08-23 My inspection found the pits were closed and the site was graded.
293864764 2014-07-08 I was on location for a urban preliminary restoration. I found during my inspection the well in production with a plunger lift unit. The tank battery consisted of a 210 barrel production tank, a 50 barrel, poly brine tank and a separator. A dike surrounded the production equipment. Identification signs were posted at the well and tank battery. A bull plug was installed on the load line. Safety equipment was properly installed on the production tank. The fencing was waived, because the well and tank battery were located in a wooded and isolated area. The lease road was in good condition. The site was graded. Vegetation was well established. The pits were closed and filled. All debris was removed. Drainage and erosion controls were in place and working properly.
-634522266 2012-07-30 On 7/30/2012 at 8:30 AM, I conducted a site visit on the construction progress for the well site. I was informed by Jason Caldwell of PAC Drilling that the Wild Cat Rig would be moving onto location today.
