1002508803 2013-06-05 On the day of my inspection H&D Drilling was drilling a 6.5 production hole on air. They were at a depth of 800'. The permit was on location, the pit liner was in good shape and there were no leaks or spills.
1369751996 2013-05-28 On the day of my inspection I found that H&D Drilling was drilling at a depth of 360' with a 10 cable tool bit. The permit was on display, the pit liner was in good shape and there were no leaks or spills.
1370007033 2013-05-30 H&D Drilling drilled 577'of 10open hole. They ran in with 7casing 20lb/ft to depth of 575'. Cemenco ran 40 barrel of gel preflush then pumped 180 sacks of regular class A cement with 10lb of celloflake. They ran a 7guide shoe and 7rubber wiper plug. They had no circulation and landed plug. Cemenco landed plug and presuured up to 600 psi. The head was leaking and we shut down. The company will run bond log and pressure test on 5-31-13 in morning.
1370016646 2013-05-31 Appalachian wire line was in to Bond log cement job with a top at 180'from surface. The rig then hooked up to air and pressured up on a swedge of 300 psi for 30 minutes. Test passed.
1370874381 2013-06-10 On the day of my inspection I found that H&D Drilling was not drilling. The permit was on display, there were no leaks or spills and there were no sags or tears in the pit liner.
1373901801 2013-07-15 On the day of my inspection I found that the hole was lost. H&D Drilling had the surface hole plugged with 6 yards of ready mix cement, from 715' to surface. They were in the process of rigging down to skid the drilling rig. I informed H&D Drilling that they will need a new drilling permit.
-1661983110 2013-07-17 I spoke with Jim Samples to inform him that he will need to submit a new plat and new permit application to skid the rig and drill a new well on this location.
-1777418870 2013-07-02 On the day of my inspection I found that H&D Drilling had drilled down to the top of the lost collar twice and then filled it back up with cement. They were on their third attempt to drill around the lost drill collar. The drill string was equipped with a conical bit on 2 tubing.
-1960378692 2013-06-13 On the day of my inspection H&D Drilling was not drilling. Mark Donaker reported that the drill string had been dropped down hole. All pieces were recovered except the last drill collar and drill bit, which was reported to be at 815'. Mark reported that they put a 200' cement plug above the lost tools and they will attempt to drill around the tools when the drill collars are returned.

884399357 2013-07-18 On the day of my inspection H&D Drilling had moved the drilling rig 12 feet to the northeast. They are waiting on a new drilling permit.
-892710459 2014-06-10 On the day of my inspection I found that the well site had been seeded and was 100% covered in well established tall grasses. I found no drilling equipment, materials or debris left at the location. This site passes preliminary restoration.
