-1701175930 2008-07-02 The Division of Mineral Resources Management received information that Mr. Larry Horn has been removing equipment from Bay State Exploration wells. An inspection revealed that the pump jack and (3) 100bbl tanks have been removed from the well. This equipment was observed during the previous inspection. Rods and tubing were observed in the well bore. The landowner states that the equipment was removed within the last 2 weeks. This well is routed to the same tank battery as the Ankrom #1 well. A Notice of Violation will be sent to Mr. Horn.
-1734411599 2008-11-18 A follow-up inspection revealed non-compliance. No action has been taken to resolve violations. Valves remain closed and gas was found to be leaking at top of well head location. New photographs obtained from this inspection. Note: Winter conditions at time of the inspection to include snow.
-537046701 2007-08-09 An inspection conducted on 8/9/2007 revealed that the well is idle. During the inspection, it was noted that there was a pump jack for this well. The electric line routed to this well has been disconnected. A tank battery was observed for this well. There are no production records for this well. This well may have been placed in the I/O program.
