1343148310 2012-07-23 Witnessed Nitrogen foam Frac job. Excavated around well casing and determined that the 7 casing was cemented to surface with slips installed. Annulus was open and exposed during frac job. No communication was observed.
1345050520 2012-08-15 Received complaint, #3949, from Mr. Roush, and inspected site based on directions from his phone call. Upon inspection we noticed a pile of oily brush behind the flow back tank but didn't notice any significant contamination. Accompanied by Morgan Button.

204728198 2012-08-15 While searching for Mr. Roush we encountered laborers from Mountaineer State Energy. They confirmed the location of complaint. Returned to the site to collect samples, and we further evaluate the extent of contamination from the back of the flow back tank down the hillside to the creek. Field samples were taken of the water in the creek and creek point bar sediment and no contamination was noticed using Quan-tabs. There was brush covered with oily material behind flow back tank. All oily materials shall be removed from site and disposed of properly. Accompanied by Morgan Button.
