1075140168 2011-10-07 On site for plug job. The operator had set the Clinton and Big Lime plug before my arrival. The operator shot the R-2, 9.5 LB pipe at 2280'. The operator obtained good circulation before each surface plug was set. This well is located at IM-0163. The operator of the mining site requested that a piece of tubing be cemented above grade to mark the well.
-1171621896 2013-07-02 My inspection found the well to be plugged. The production equipment has been removed. The area where the well was located is being used as a storage yard for construction equipment and materials. Final restoration is passed.
1185978233 2007-07-31 Well is shut in. Well head is under 3' of cement as well as the pump jack base is under cement. Area is Sidley's cement truck clean out area. Well needs to be plugged or repaired and put back into production. Tank battery is in capable of operation. Separator is 50% under cement. Tank is cemented in. Called Bob Sidley left him a message to call me regarding well.
1291924730 2010-12-09 Well is equipped with a pump jack. Wellhead is under approximately 4' of cement. The well location is under approximately 3' to 4' of cement. Tank battery is still in place and is starting to be covered by cement. This well is incapable of production. The well needs uncovered and put back into production or plugged. Well needs to be plugged by 03/09/2011 or further enforcement action will be taken.
-1723540823 2011-10-03 Mining operator has chiseled well head out of the waste cement that had been covering it. Well is exposed for plugging operations.
1845959409 2009-03-20 Met with Al at Sidley's scale house. Company has been hired to plug well. Company will take care of obtaining permit. Well is incapable of production.
