1225276271 2010-12-29 I witnessed the fracing of the Clinton zone. Universal preformed the frac with Tom Wood as the Everflow Eastern representative. The Clinton zone broke down at 1465psi. The average rate was 46 BPM with the average pressure of 2200psi. The ISIP was 953psi. The 5 minute reading was 884psi. There were no violations at this time.
1292270568 2010-12-13 I performed a rig inspection on Wildcat rig 1. The rig crew had the drill pipe out of the hole and ready to make a motor run for the directional start. Heavy snow. The drilling depth was 1996' on air. This inspection satisfies 1501:9-9-03. There were no violations at this time.
1292850324 2010-12-19 I witnessed the cementing of the 4 1/2 casing. Full circulation throughout the cement job. There were no violations at this time.
1293728562 2010-12-30 I partially witnessed Everflow solidify the drilling pits. There were no violations at this time.
1301423849 2011-03-24 I received a phone call from Sean McGuire-Urban Conservationist Mahoning SWCD (330) 740-7995 about some dirt that is close to a creek. The top soil was spread last December around the drilling location. I told Sean that I would call George Strawn from Everflow Eastern about getting the creek area cleaned up. I would meet him next week. I was on vacation. I called George Strawn(330-727-1614) about this matter. He went out to inspect the area and called me back. The dirt had not ran into the creek. I asked George if we could meet next Tuesday, 3/29/11. I emailed Sean about this meeting. He will bring Anna Mamone (Boardman Zoning Inspector) to the meeting on Tuesday. This complaint continues.
-1392598558 2011-08-04 Chain link fencing installed around well head and tank battery. ID sign posted. Top soil graded out, vegetation well established. All drilling equipment removed from site. This well site satisfies ORC 1509.072. Preliminary restoration passed. No violations at this time.

-1885441797 2011-04-04 I inspected the drilling location area and found the area seeded, strawed and a silt fence placed on the northwest to the north side of the drilling location. This complaint is resolved.
2001153478 2011-03-30 I inspected the progress on the area. Dan's Excavation Service were taking out the tree stumps and dirt from the creek bank. The dirt will be sloped so there will be no debris going into the creek. This complaint continues.

-363140289 2011-03-29 I met with George Strawn(Everflow Eastern), SeanSean McGuire-Urban Conservationist Mahoning SWCD,Anna Mamone (Boardman Zoning Inspector) . Also present was the supervisor of the Mill Creek Commentary and inspector Chris Grimm. We all were in agreement to take some of the dirt on the west side of the drilling location and bank the side so that the dirt erosion would be minimal. George will start on this work tomorrow, 3/30/33. This complaint continues.

-750621489 2010-12-14 I performed a rig inspection on Wildcat rig #1. The depth was 3137' and on air. Heavy snow. This inspection satisfies 1501:9-9-03. There were no violations at this time.
-825757090 2012-01-27 I conducted a status check on this site and found that the well head was fenced with chain link and it is equipped with a plunger lift system. The tank battery consists of one 100 barrel stock tank and six separators. There was no brine storage tank. The thief hatch was closed and there was a bull plug installed in the draw valve. The tank battery was fenced with chain link and the dike was free of vegetation and debris. The tank battery is shared with permits 2619, 2742, 2200, 2777, 2536 and 2666. Proper identification was posted. I found no leaks or spills at the well head or the tank battery. I took new GPS coordinates of the well head and tank battery. No violations at this time.
