Tank battery for this well and the two other Mid State Basel-Leach wells, #1 (Permit Number (P/N) 20881) and #2 (P/N/ 20880) is located in a dike containment area next to the asphalt entrance road from CR 1 to the Mid State Property. There are three 100-barrel tanks. Two tanks are connected together at the top. These tanks are gathering oil from the Mid State wells. The identification sign on the one tank has incorrect information. According to 1501:9-9-05(A)10/1509:23 well identification sign shall include owner's name, county, township, well name and number, permit number for the well, and emergency contact phone number. The tank shall also provide signs of indicating flammable hazard with the appropriate National Fire Protection Association Standard sign indicating this and other hazards. There is two gas-fluid separators each followed by a small drip separator and then separate gas meters. The dike for the containment area has a two inch pipe at the floor of the containment area to allow accumulating rain water to drain from the area. The containment area has some petroleum staining. Water from this containment area plus escaping oil and brine could quickly reach a wetland area adversely impacting this environment. According to 1501:NONE(B)/1509:22 the pipe shall be removed and the hole filled in so no liquids leave this impoundment. All water from the containment area shall be collected in the 1100-gallon poly tank, used for brine collection, also in the containment area, for later disposal. The wellhead is a pneumatic lift well. The identification sign has the incorrect information. As with the tank battery for these wells, at the wellhead according to 1501:9-9-05(A)10/1509:23 the well identification sign shall include owner's name, county, township, well name and number, permit number for the well, and emergency contact number.
