Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1000767226 2013-10-02 The producing unit consists of a pump jack unit at the wellhead and an aboveground storage tank (AST) battery. The battery is comprised of 1 100-barrel steel production tank and a separator unit. The elements are enclosed in separate chain-link enclosures. There is adequate secondary containment and signage. No problems noted. The battery is shared with the #882, and #884 units.
-1219205171 2012-04-25 My inspection today is to follow up on a complaint from the mayor of Perry Village. The operator changed the well from a plunger lift system to a pump unit. The land owner south of the well, has a patio and sun room that faces the well. The neighbor had inquired about a visual barrier. The operator of the well installed a wood visual barrier wall. The wall blocks the view of the pump unit for the neighbor.
-1337553467 2012-02-09 The rig is tripping tubing back into the well. The operator will be installing a pumping unit and is going to fence well with board on board fencing.
-1560591429 2012-03-29 The Mayor of Perry contacted me in regards to a visual barrier for this well. This well was converted from a plunger lift system to a pumping unit. The landowner, who purchased the adjoining lot south of the well, built his house with the sun room windows facing directly at the well. The well has been fenced with new 6' chain link. I spoke Ron Gibson, who will install a section of 8' board on board fence to block the the view of the well from the home. I spoke to the landowner immediately west of the well and he stated that the well is not an issue for him as his shed blocks any view of the well. The Village of Perry does not allow buildings within 300' of a gas well. I notified the Mayor, who asked for my opinion on the village spacing rules, that ORC 1509 requires 150' spacing for an inhabited structure to a wellhead in urban areas, unless the landowner approves a waiver for 100' and for unoccupied structures, there are no rules.
1943189469 2014-11-10 I inspected this location has part of routine inspection activity. The well is producing and production records are current. The fittings and connections at the pump jack are free from signs of leakage or other problems. The chain-link enclosure is intact and locked. Fittings at the tank battery are free from signs of leakage or problems. The load-outs are bull-plugged and locked. Safety equipment is present. Secondary containment is intact. ID Signage is present. The lease road, shared with the nursery facility, is comprised of hard-packed dirt and is dry and stable currently. The battery is shared with the #2-0882 and #2-884 units.
