-1769949417 2012-11-30 I completed an urban preliminary restoration inspection. My inspection found the well equipped with a plunger lift unit that was surrounded by a chain link fence with slats. The tank battery was surrounded by a chain link fence with slats and contained one, 100 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and an identification sign. Lease road was next to Petsmart. The production equipment was missing a charcoal filter, low pressure relief valve, and lightning arrestor. I called Matt Kleese at 330-360-8055 and asked for the required safety equipment to be installed. The pits were filled and debris removed. The site had vegetation growing.
-1802182428 2012-12-28 My inspection found the lightning arrestor, low pressure relief valve, and charcoal filter installed at the tank battery per my request. This well meets the urban requirements for Senate Bill 278.
290274982 2012-08-27 On 8/27/2012, I completed an Urban Preliminary Restoration inspection. My inspection found the well equipped with a plunger lift unit (rabbit) that was surround by a chain link fence and in good condition. The tank battery was surrounded by a chain link fence and contained one 100 barrel oil storage tank, one 50 barrel brine storage tank, a separator, and the proper identification. The production equipment had the proper spacing, the pits were filled, and all debris removed. The well site was graded and seeded to prevent erosion and sedimentation. I noticed no visible spills or leaks. The lease road was in good condition. There are no violations at this time.
-438798334 2015-02-24 I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a plunger lift assembly. The tank battery consisted of a 100 barrel production tank, a 50 barrel brine tank and a mechanical separator. A dike surrounded the tank battery. An identification sign was posted. There was a plug on the load line. Locked, chain link fences with slats surrounded the tank battery and well. Urban safety equipment was installed on the production tank. The lease road was in stable condition.
