1061562688 2012-10-27 Arrived on site to relieve local inspector he had been on site overnight trying to bring well under control,Corp Oilfield Services had 7 company men on site and local fire department had pump truck on site and 1 mud engineer and formation cementing had 2 company personnel on site mixing mud and weight material so they could get well under control, they mixed up slurry to 12.5# per gal at a volume of 130 bbl and we pumped it down annulus of well to kill H2S gas once it was determined that sour gas was under control they then lit remaining gas vapors and then decision was made to make plans on plugging said well.

1351442119 2012-10-31 On the day of my inspection I witnessed the removal of all the tubing and fourteen hundred feet of the four inch production string, I also witnessed the plugging procedure.
1351442222 2012-10-26 After receiving a complaint phone call from Shirley Ball at 58217 Vocational Road Byesville Ohio 43723, stating that she was smelling a strong oder of gas that was making here sick from a near by well that belongs to Northwood Energy. I call inspector Eric Conrad to meet me at the lease road of the well site, upon arriving I found Ron Corp well services at the well site working on the leak at the well head. I asked the crew if they had an H2S detector with them and they replied yes they did and showed it to me, it was not reading any H2S presence where they were working at. I then took a reading with my detector at a distance of ten feet and got a reading of 17 ppms up to 45 ppms, those numbers stayed the same through out the evening and morning. After talking with the crew and getting the H2S readings I then called the Guernsey Co. Sheriffs dept to have them dispatch the Byesville fire dept to the site, the fire dept stayed at the site while all attempts were being made to kill the gas leak. A decision was made since the fire dept was there to flare the well to help get the gas pressure down on the eight inch so the two inch nipple could be welded on the side of the eight inch where the leak was coming from. The flare burned for an hour but there was no decrease in the back side psi, Bill Arnholt called Producer services to come with a pumper and pump into the four inch to get the psi down, once the psi was down on the four inch they then pumped into the eight inch to try to kill the psi. After pumping into the eight inch for a short period of time the psi was increasing, a decision was at that time to stop pumping and flare the eight inch to bring the psi back down. Bill Arnholt made a decision to bring in Formation Cementing and try to kill the well with drilling mud. I arrived at the well lacation at 9:30 am on 10/26/2012. The companies on site were:ODNR/ Ron Corp/ Producer services/ Formation Cementing/ Byesville Fire Dept. The well owner is Northwood Energy.

-1471517364 2012-10-30 On site to witness remaining plugs set in well the pipe was parted@1405' and decision was made to set plugs from that point we sat a 300' at 1400'-1100' and had good circulation and then pulled up to 300' and set plug from that point to ground level and it dropped 30' and they topped it off with 10 sacks cement. Well was dug out to approximately 6' below ground level and steel plate attached with permit number on it and site roughed in awaiting better weather to do restoration work.

Around 7:30 AM on Saturday the 10/27/2012 I called Dan Goins to give an up date and a decision was made with approval to switch out with Dave Ball at 10:00 AM on 10/27/2012.

-438205353 2012-10-29 On site to witness setting of lime plug casing was perforated at 3860-3850 by Appalachian Well Surveys and then rig crew ran tubing back into well to set required plug and then tubing was pulled out of hole to prepare well for next required plug.
622229469 2014-07-17 On the day of my inspection I witnessed that the site does not pass final restoration according to ORC 1509.072B. A skid of Bentonite and 3 inch pipe remain onsite. I made a call to Bill Arnholt and advised him that this needs addressed immediately.

717742644 2012-10-26 I met inspector Hartshorn in Byesville. We drove to location and met with the fire dept. that was on site. There was a strong smell of H2S at the end of the lease road. Upon arrival we found Corps Well Service trying to hook up a flare line. They had no monitors or safety equipment. We requested that they get safety equipment on site before proceeding. Once their monitors arrived they proceeded to hook up the flare line, and began to flare the gas. After 30 minutes the pressure on the 8 did not seem to drop. A check of the pressure on the 4.5 showed that there was communication between the strings. Corps Well Service called a water truck to the site to kill the well, but the pressure was to high. Producers was then called out. Upon their arrival I left the site.

946126913 2012-10-28 On site along with Rick Buxton and crew to unseat down hole pump and once unseated Formation Cementing pumped water down tubing to ensure that they could possibly pump bottom hole plug once rods and pump was removed from well.

Formation Cementing set 650' plug 55 sacks of Poz mix cement 15# and 1.08 yield then we shut down for the night and flare was still burning and company had personnel on site throughout the night until operations began again in the morning.
