1498217972 2012-06-14 On 6/14/2012, I conducted a production well inspection status check. My inspection found the well was a plunger lift unit (rabbit) that was in good condition. The tank battery contained one 210 barrel oil/brine storage tank, a separator, and there was no identification posted. The lease road was in good condition. I called Jay Gas & Oil (330-652-1749) and asked to have the proper identification posted.

525725377 2015-02-05 I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a plunger lift assembly. The tank battery consisted of a 210 barrel production tank and a mechanical separator. An identification sign was posted. There was a dike around the production equipment. A plug was installed on the load line. A production meter and 7 day chart were installed inside the separator. The lease road was in stable condition with a gate at the entrance. The well was placed into production prior to House Bill 278.
-75290465 2012-07-26 On 7/26/2012, I completed a follow-up inspection status check. My inspection found the proper identification sign was posted at the tank battery and a new gate was installed at the entrance of the lease road. Inspection passed.
