-2051967390 2004-03-29 Restoration approved .
214954749 2003-07-15 Meets requirements for preliminary restoration. The well is shut in. The tank battery consists of 2-100 bbl storage tanks and a separator. The dike is eroded away on the east side and is breeched with a plastic pipe.
-38811525 2015-03-13 I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a pumpjack. The flow line ball valve was in the open position. The tank battery consisted of two (2), 100 barrel production tanks and a mechanical separator. There was a dike around the tank battery. An identification was posted with the J Well as the well owner. Plugs were on the load lines. The lease road was in stable condition. The well was placed into production prior to House Bill 278. I called Jim Adams of J Well. Mr. Adams states D&L had transferred the well to the landowner. Also, Mr. Adams claimed the landowner had a copy of the Form 7. I informed Mr. Adams that the landowner needed to file the Form 7, along with the Form 7 between the landowner and J Well immediately. Mr. Adams stated he would immediately correct the issue.
