1055900879 2015-01-29 I inspected this producing unit as part of routine inspection activity. The wellhead pump jack enclosure is secure. Valves and connections are secure and free from signs of leakage or problems. At the time of my inspection, the area is covered with several inches of snow. The dedicated access road has been plowed and is passable. The tank battery which is shared with the #2-2147 unit is secure. Valves and connections are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.
-1071784235 2012-11-28 My visit to the well today was to perform an Urban Preliminary Restoration inspection. The access road is concrete for the first 30 feet and then packed crushed limestone and brick for the remainder of the well site. The wellhead is enclosed by a board on board fence and the gate is secured by lock and chain and is secure. No problems or leaks were noted at the well head. The tank battery is a shared location with the Callis 5 well, P# 2-2147 and the gate is secured with lock and chain. The dike surrounding the tanks is sufficient to hold fluids per 1501:09-9-05. There are two 140 BBL tanks for this well with caps closed and secure with vents and a charcoal filter in place. There is one 100BBL brine tank which is shared by both wells. This well has one vertical and one horizontal separator. Load lines are enclosed within the tank battery and are secured with locks and bull plugs. Urban Preliminary inspection passed.
1405007673 2014-07-10 I inspected this producing unit as part of routine inspection activities. The wellhead which is currently fitted with a pump jack system is in operation. Production reports are current. System operation at the above-ground storage tank (AST) battery appears normal. Safety apparatus are present and I observed no signs of leaks, spills, or problems.
-1688362030 2013-04-03 The producing unit consists of the following elements: a production wellhead and a storage tank battery. The production wellhead is fitted with a pumpjack system and is currently producing. The pumpjack unit is enclosed in an 8-foot board-on-board enclosure which is locked. The tank battery consists of 4 100-barrel steel tanks, 1 100-barrel steel tank, and 2 separator units. The battery is enclosed in a board-on-board enclosure. Safety equipment (carbon filter, spark arrestor, low-pressure relief valve) is present. The secondary containment is present and in good condition. No violations noted. The tank battery is shared with the Callas 5 Unit.

The well and tank battery are located in cleared, but undeveloped parcel in an otherwise residential area, adjacent to the house at the referenced address. Fittings and connections at the wellhead pump jack and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present. No problems noted at the time of this inspection.
