1259235552 2014-05-14 Well is a producing well with Pumping unit powered by natural gas powered single piston motor. The rod annulus is open for oil production and the tubing annulus is open producing gas for pumping unit motor. There are 2-100 barrel tanks that are leaking at burner tubes and the tanks are decomposing at the bottom. There is a vertical separator that is not connected. There is large separator on a stand that is leaking brine down into containment. Quantabs were used that revealed the Chlorides were above 6775 parts per million. All the piping plumbed into the separator and tanks are leaking and decomposing. The containment that encompasses all equipment is measured by 96' in length and 24' in width. The dike wall measures at 3 'across at the fullest point. The dike has been breached by a decomposing pipe and valve and by migration thru the containment walls. There was a brine scald area that measures at approximately 39' x 34'. Identification is placed at the tanks but is unreadable. Remove all tanks and plumbing. Remove all contaminated soil and haul to landfill. Replace soil with good top soil. Reseed and mulch as per Best Management and Practices. Replumb and install good tanks. All manifests will be provided to the inspector upon request. Respond to call made by Patrick Shreve by 24 hrs. Company will have 48 hrs to resolve upon receipt of Compliance Notice. Failure to respond will incur in further enforcement action.
1401983028 2014-06-05 I met Clay Johnson and Mark Hines at well site. They have a heater tube dripping and are going to have a welder come out and weld it. They will notify me when the welder is done. We also talked about getting other wells in the vicinity where the lines are not marked to identify them.
-1510898055 2014-05-19 The tanks have been picked up and moved and replaced by one 100 barrel tank. The location has been cleaned up. The tank has been diked up. They are still working on site. The contaminated soil has been hauled to Killbarger landfill.
-1686095644 2014-05-20 Mark Hines called me on the status of the site. They are done. The pumper will install the identifications at the tanks. There will be a total of two haul tickets for the Killbarger landfill. He will provide at later date.
432700908 2014-05-29 I stopped by and looked at the well and tank. The well and the tank have containment around them. The tank that was brought in is leaking thru heat tube. The Compliance Notice did not pass on the day of inspection. I called Mark Hines and he would have welder come in and weld shut.
