1152654220 2006-07-11 Called by Steve Harley to report an oil spill. Service rig was cleaning out well into lined pit. Heavy rains came during the night and flooded well location. 5 barrel of crude oil in pits was carried down small stream to the Mohican River. Crude oil flowed in corn field east and south of the drilling pits. Called OEPA, Wes Drake, and he arrived on scene to supervise river cleanup. I instructed Steve Harley to pump pits as soon as possible due to more rains coming today. Booms were installed in stream leading to Mohican River. Crews are vacuming oil from water. Pits were pumped. Humid, 80 degrees.
1152884805 2006-07-14 Oil spill clean up continues in channel and corn field. Flooding of low lying area continues. 9 of rain fell 7-11 and 7-12-2006.

1377116252 2013-08-21 Requested by Michele Miller of ODNR to inspect the well. There was an NOV from 2006 during well drilling that was not terminated. The violation was for drill pits overflowing oil during flooding. The violation no longer exists and the NOV may be terminated. The stuffing box at the well is leaking and needs repaired. ID needs to be posted.
271855406 2005-09-19 Called by Steve Harley to witness surface casing cementing. Permit expired on 9-7-2005. Steve Harley advised the well was spudded on 9-7-2005 but mud pump troubles kept them from setting pipe earlier. I advised Harley Drilling to notify State inspector in the future or a new permit will have to be applied for. He agreed. This permit was reissued in Sept. 2004.
