INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1149793328 2014-03-18 A well head, 4.5 in. casing, swage, a 2 in. valve, a nipple, and another 2 in. valve can be observed from the surface. The second 2 in. valve is so heavily corroded that there are no recognizable threads on it. Coming from the well head is a steel nipple and a 2 in. valve. It is corroded in the same way. There are no production lines connected to this well. There haven't been in a long time. This was noted due to the condition of the valves. The tank battery consists of 1 100 barrel tank and a separator. Lines connected to the separator are also heavily corroded rendering it inoperable. There is orange plastic line that goes from the tank battery, down the lease road to a meter run GPS coordinates are N 39.40102, W 81.32696. There are two noticeable breaks in the line observed from the lease road. One of them is 12 ft. away from the meter run. There is no meter in the housing and there hasn't been in a long time. This was noted due to the corrosion in and on the meter housing. New pictures were taken. Compliance Notice

-1231375708 2014-08-29 Upon inspection, it was observed that no effort has been made to bring this well into compliance. The well consists of a 4.5 casing with a swedge to a 2 valve. The well is badly corroded, but no leaks were detected. The tank battery consists of a mechanical separator and a 100 bbl tank. The production equipment on this location is not usable. Previous inspection reports indicate that Stonebridge Operating has denied that they own this well. I will contact Mr. John Schneider of Stonebridge and inquire about the well.
