| 1144870925 | 2006-04-12 | My inspection this day found this well not in operation. The 4.5 production casing was hanging on a clamp. I was told that this well has not been cemented. No identification signs are posted. I called Larry Haynes who said he will plug well in the next 12 months. I agreed with his time frame and gave him a verbal Notice of Violation (NOV) at this time. 10/8/07- No change in well status, so I will issue a written NOV to produce or plug this well and post proper identification signs. Failure to comply may result in bond forfeiture. 6/10 - I contacted Jim Zimmer, now supervisor at A&R Energy, who requested the paperwork to file for a plugging permit. He plans to plug the well this summer. 12/6 - I spoke with Jim Zimmer who said he did not get the plugging permit filed. 1/20/12 - Larry Martin reported he has bought the shallow wells of A&R. He said this well probably will not be plugged until summer. | ||||
