1162484959 2006-11-01 Complete and valid permit on site at the time of inspection. Pits are in good condition. Reminded owner/operator that, per the permit conditions, all fluids shall be removed from drilling pits within 15 days after the completion of drilling. Pits shall be closed within 30 days after the completion of drilling. Drilling had started at 3:25 PM. Operator was drilling out cement from the 8.625 casing. Notice of Violation has been passed. Operator has posted a valid permit on site. Failure to follow permit conditions at this time are non-remedial. Failure to follow permit conditions in the future may result in suspension of future permits or a consent agreement with the DMRM.
1163706879 2006-11-16 Operator is in the process of solidifying drill pits. 3 loads of cement dust has been brought onto site, operator has two more coming. Operator will be removing additional water from south pit.
1668512578 2012-04-06 My inspection found the well and tank battery in good repair.
1680885585 2006-10-31 On site to check the status of the site. Operator contacted me regarding his failure to meet permit conditions. Permit #1815 had requirements that the Inspector be contacted 24 hours before site constructions as well as pit construction requirements. I was not contacted before site construction. At the time that I was contacted pits were constructed and drilling was active. Special permit conditions attached to permit were regarding thin glacial till overlying fractured bedrock. The material excavated for the pit construction was clay with siltstone. In the future, the operator needs to follow permit conditions in regards to contacting inspector. Operator needs to confirm that a valid and complete permit is on site before drilling is started.
825772435 2007-09-04 Farmer is planning on returning well area back to a hay field, site has been graded and is stable. Tank battery has been installed, one 210 and one 110 bbl tanks on site. Mechanical separator on site. Well is a pump jack well.
