1250159758 2009-08-12 I was called out on 8/11/09 at 10:00PM by Kurt Kollar of the Ohio EPA(216-789-9282. Lightening had struck the stock tank and blow out the bottom third of the tank. The Ellsworth Fire Department were on the scene. I tried to find an ID sign on the stock tank. It was only half there. I finally called Dick Heldman ( 330-807-4224) who was the pumper contractor for Samurai Operating Company. I needed the well shut down. Mr. Heldman did come out and shut the well in.

Mr. Larry Bynum from Houston(713-771-5500), Field Superintendent, called to make sure everything was going good. EPA had the spill contained with 4 booms in the North Fork Creek. Mr. Kollar decided to wait till day light before we start the clean up. I arrived at the site and found that there was no dike. I called Mr. Bynum to let him know that there was no emergency phone number on the ID sign and there was no dike around the stock tank. He agreed to install a dike when the repair work is finished. The clean up is at 80%. The 100 tank had less than 3 barrels of oil in it. Less than 10 gallons went into North Fork Creek. The paper and TV reported that the spill was 10,000 gallons. This report was not correct. This case continues.

1250706844 2009-08-19 The dike and the 50 barrel plastic stock tank were installed. The ID sign was posted. This NOV is resolved.
-1780184640 2009-08-13 The cleanup is 95% done. The dike is up and the new 50 barrel plastic stock tank has been installed. The diapers and booms are still in the creek. The old dirt and oil from the dike has not been hauled away yet. This case continues.
