Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1147950131 2004-03-12 An inspection of the well revealed no ID at the well head or tank battery, production tanks leaking. Field Visit or Inspection
-1147950131 2004-03-12 An inspection of the well revealed no ID at the well head or tank battery, production tanks leaking. Compliance Notice
1244033733 2009-05-21 An inspection revealed that a line located on the bottom of the saltwater tank is leaking brine onto the ground surface. The line shows signs of corrosion and the source of the leak is around an installed band clamp located approximately 6 inches away from the valve. Erosion was observed around the lease road and well site. The well head location will require an earthen dike and the dike walls around the tank battery are eroding and will need to be addressed. The displayed identification incorrectly shows Johnson Oil and Gas, as the well owner and not Mike Johnson. Furthermore, the identification is fading and will need to be replaced. Note: A service rig may have been placed on location, as area located near the well contained old oil breaking down (steaming of rods and tubing).
1268373665 2009-07-06 A follow-up inspection revealed that the band clamp was not leaking at time of the inspection. The lease road has been graded. Contamination and pollution still exists inside the dike and ID not corrected.
1269i6 2001-01-16
-1273463193 2009-06-18 A follow-up inspection revealed non-compliance. 2 black plastic routed from well to a second 100 bbl tank with no kncokout tank. The steel line to leaking tank not connected to the well. Two leaks observed on both sides of the band clamp
1401129944 2009-07-22 A follow-up inspection revealed that old tank has been removed. A used 100 bbl tank has been installed with tar along bottom lip of tank. The ID has been corrected. Fresh oil observed around well head appears to be from stuffing box leak. Left a note for Mr. Johnson stating cleanup fresh oil, check stuffing box rubbers and recommend addressing dike wall (front) which has eroded.
