Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1372362012 2013-06-27 On this day I witnessed the cementing of 1093' of 13.375 surface casing with good cement returns to surface and the casing pressure tested to 700 psi upon the landing of the plug. The mix water was tested and showed no chlorides and a PH of 7. All records received - casing tally and cement tickets.
1389401884 2014-01-10 Status check inspection found the 12.250 2nd intermediate well bore section total drilling depth has been reached at 8,272' and while pulling out of the hole with the drill string the assembly became stuck at 7,230' and was backed off with 160' of the assembly being lost in the hole, Blue Dot recovery service was on location trying to recover the assembly.
1390874158 2014-01-27 On 01/27/14 8,274'of 9.625 casing was set and cemented to surface with a 2 stage cement tool @ 4,011', a lead and tail cement was ran on the bottom section and a single cement mixture ran on the top section, all cement used was Type 1 cement, a seal bond spacer was ran ahead of the cement. All records received.
1391461644 2014-02-03 Status check inspection on the drilling progress found the current drilling depth to be 11,112' using a 8.5 directional drilling assembly with the current borehole azimuth position of 303.5 and inclination of 91.45 degrees with a 13.# mud weight and 90 viscosity.
1392063831 2014-02-10 On 02/09/14 the production casing was set at 14,773' and cemented by Baker Hughes using a lead and tail cement mix with cement returns to surface and the casing pressure tested to 4100 psi which was 500 over lift pressure of 3600 psi, a rapid start valve was ran in the casing string to once again check the casing before beginning stimulation. All records received and the well bore drilling process is now complete.
1417962547 2014-01-02 Status check inspection found the current drilling depth to be 4,280' in the 12.250 1st intermediate well bore section with the last well bore deviation survey taken at 4,280' showing a deviation of 1.58 degrees and direction of 209.48 degrees with a true vertical depth of 3,985.35'.
-151001420 2014-01-06 Status check inspection on the current drilling progress found the current drilling depth to be 7,128' using a 12.250 hammer bit with the drilling rig pulling out of the hole with the drill string for a bit change.
210458812 2013-12-11 The said well is in line and producing with approximately 7 barrels of water being produced per hour. 3 wells on location are now on line with 2 more to be hooked up and on line today.
-338166959 2015-02-26 Preliminary Restoration status check inspection found a production tree present at the well head with the production valve in the open position with a flow line leading into a gas processing unit. There is 6-200 barrel capacity production tanks located inside a mechanical dike area. Preliminary Restoration passed on this day.
552863588 2013-05-14 The said well was spudded on this day with 80' of 3/8 wall 20 diameter conductor being set with no annular space.
-637015396 2014-02-06 Status check inspection on the drilling process found the current drilling depth to be 11,783' in the horizontal well bore section with a directional azimuth of 300.45 degrees, the total drilling depth shall be 11,783'.
721331430 2014-03-19 Status check on site inspection to witness the 5.5 production casing pressure test, the casing and well head assembly was tested as one part on the low pressure side at 500 psi, the well head lower valve was the closed and the well head pressure tested to 9995 psi, the pressure was the bled off and the main valve opened and the casing tested in 3 pressure increments beginning at 1500 psi then 2500 psi and a final pressure of 7700 psi and the pressure was held for 15 minuets with no recorded loss, the casing toe sleeve valve was the opened by increasing the pressure to 8700 psi completing the test procedure.
-948951043 2014-01-21 Status check inspection found HAD drilling rig 105 on the hole with a total depth drilled of 6956' in the 12.250 1st intermediate well bore with the drilling rig pulling out of the hole for a bit change. A 559 sack class H cement kick plug was set over a lost drill bit from 7175' to 6650'. All records received.
