Uphill from the well is a plastic brine storage tank that is falling in from the top due to compromised integrity from weathering. It is half full of fluid and there is no dike around it. Production records have not been submitted for the year 2011. Separator needs to be moved to an appropriate distance from the well. Complete identification needs posted at the well. All pollution and contaminated fluids and soils need to be removed and disposed of at an EPA approved landfill. The plastic storage tank needs to emptied, contents properly disposed of and removed. Production records need submitted. Failure to comply will result in forfeiture of bond. Accompanied by Southwest Region Supervisor Dan Goins.
