1264502701 2011-08-10 Top soil graded and there is well established vegetative cover. All production equipment has been removed. The lease road has been left in place per landowner request. Final Restoration passed.
1299266066 2011-03-04 Chris Grimm and myself witnessed the plugging of the Clinton, Big Lime, Berea and surface. The tank battery was removed some 4 years ago. Universal preformed the plugging. Appalachian Well Survey preformed the bond log and cutting off of the 4 1/2 casing. The cementing from 250' to surface did not show visual signs of cement or cement water. Will tag cement on Monday. There were no violations at this time.
1299785916 2011-03-07 I witnessed the tagging of the cement by Summit well service. The top of the cement was at 95'. Russell Miller from Vanguard proceeded to call Salem Ready Mix to come out with 1 1/2 yards of 9 sack grout. Ready mix came on the location and dumped 1 1/2 yards of cement. Chris Grimm also witnessed the cementing from 95' to 0'. Used 1 1/2 siphon tube to extract the water. They will cut off the casing and fill in the pit tomorrow. There were no violations at this time.
499118595 2011-03-08 Chris Grimm and myself witnessed the cutting off of the 8 5/8 casing, installing an ID sign on top. There were no violations at this time.
877376110 2011-03-09 The pit and the area around the wellhead were closed. Some of the debris was removed. There were no violations at this time.
945299812 2011-03-30 The area around the wellhead is rough graded in. No seeding has been preformed yet.
