An area around the wellhead and along the uphill and downhill sides of the site is void of vegetative cover. The owner or the owner's agent shall grade or terrace and plant, seed, or sod the area disturbed that is not required in production of the well where necessary to bind the soil and prevent substantial erosion and sedimentation. Failure to comply with in the given time period will result in a material and substantial violation.

Took photos. Informed Tom Stone Mountaineer Field Supervisor of the leak, and instructed him to fix the leak and clean up the oil.
757969285 2012-04-27 There is erosion running down the hill roughly 120 feet. No vegetative cover has been established. Area shall be graded and vegetative cover shall be established in order to prevent erosion. There is a bailer on site next to the well. All drilling equipment shall be removed.

840269341 2011-09-20 John Kent called and scheduled a Annular Disposal Mechanical Integrity Test (Fresh Water Test) on the surface casing. Test pressure 343 psi. 1st test (at 345 psi) lost 25 psi in 5 minutes. 2nd test (at 355 psi) lost 75 psi in 15 minutes. No further tests were done.
-868784687 2011-09-20 Surface cement job. Ran 682 feet of new 26 lb 9 5/8 inch surface casing in 690 feet of 12 1/4 borehole. 265 sacks of class A, 300 lb gel, 800 lb accelerator, 240 lb LCM, with 60% excess. Circulation established and good cement returns stayed at surface.
974494694 2011-12-21 Pits filled for two month preliminary restoration requirement. Some areas seeded and mulched, not all. Tank battery has been placed 50 feet from road. Well in production but ID has not been placed. Spoke with Tom Stone about ID.
