1149106003	2006-05-31		Dave Hodges requested a water sample from John Cutlip's water well at 1500 Bell Rd, Wooster, Oh. 44691. Dave Bishop is rigging up on an abandoned gas well leaking gas near Goody's home and 50' from water well. History of area indicates diminution problems. Collected a water sample from Cutlip's 2 water wells and sent to Cambridge lab. Wells are hooked together to supply water to home. #1 well is 240' and # 2 well is 160'. No odor or filtering equipment attached to system. Advised Bishop to begin drilling out gas well. 75 degrees, sunny.
1149262698	2006-06-02		Requested by Dave Bishop to inspect drill site and BOP before drilling. Decision was made to run 7 casing to 300' and cement before cleaning out well or bridge. Water well area with in 20' of old gas and oil well. Emergency drill out and plug.

No injuries or home damage. S&H Water Service arrived with 2 trucks and pumped steel pits. I notified John Cutlip that well blew out and is under control. Loaded well with drilling mud and ran drill pipe to next bridge. Cloudy,75 degrees.
