1360594875 2013-02-11 My inspection found the well equipped with a plunger lift unit and a domestic gas line. It was shut-it on the production side. The tank battery contained one, 100 barrel production tank, a buried, concrete brine tank, and a separator. The dike was in good condition. There was a lock and plug on the load line. The production records on our database show zero for more than two years. The lease road was the landowner's driveway. I called Larry Lower at 216-570-8708. I informed him to plug, produce, or file for Temporary Inactive Status by 4/1/2013.
1368623493 2013-05-15 Enervest Operating filed and was issued a plugging permit on 5/8/2013 per my request from an inspection on 2/11/2013.
1429702317 2015-04-20 I was on location for an exempt domestic well inspection. During my inspection, I found the well in good condition. The well is equipped with a plunger lift assembly. The main flow line valve is in the closed position. A 2 inch domestic gas line teed off of the 4.5 inch casing. The tank battery consists of a 100 barrel tank and a mechanical separator. A dike surrounds the tank. I spoke with the landowner, Mr. Burgess. Mr. Burgess owns 24 acres. The well is located on Mr. Burgess' property. The well supplies domestic gas to his residence. The well is over 200 feet from all private and public buildings.
-1751871648 2013-06-06 My inspection found the well not in production. The well was equipped with a plunger lift unit. The flow line ball valve was closed. The tank battery contained one, 100 barrel production tank, a buried, concrete brine tank, and a separator. It was surrounded by a dike capable of meeting containment requirements. There were plugs on the load lines. The proper identification was posted. The lease road was in good condition with a locked gate at the entrance. According to our database, a plugging permit was issued 5/8/2013.
