1017456852 2009-09-25 Ran integrity test on tubing to 2000# for 15 min. - 0# on annulus - no change in pressure. Then proceeded to test casing - pressured up to 2000# for 15 min. and it held. Next, J-d back onto packer, circulated corrosion inhibitor, then lastly, opened valve in packer so well could inject water.
-1057901708 2012-11-20 Inspection revealed that injection pump was in operation during my inspection and facility was in satisfactory condition this inspection.

Note: Positive pressure needs to be applied to annulus of tubing so we can ensure you have good integrity of production casing.

-1089966103 2010-02-24 Inspection this day found well shut in, not taking water this day. Chart on well shows tubing pressure at 1250#, 200# on annulus and holding steady over the last week or more. Not enough water to use this well right now, weather too bad to haul water.
1102231050 2009-08-31 Inspection this day found Arvilla, serive rig on location, swabbing water off the well. Tomorrow, acid will go in hole and well will be perf'd. Bringing rock in, frac tanks in. Drill pits are partially empty, all drill equipment removed.

1266765192 2010-02-11 Bruce Phipps reports that they have not been using well due to low water volumes. Reports they have 1400# on tubing, 200# on annulus and have charted it for last week or so with no changes. Both pressures holding steady
1266966439 2012-03-29 On site to witness acid job preformed on well Producers Services pumped a total of 2860 gal of water and acid down well to clean up formation they pumped .7 bpm with a average pressure of 1938 psi.

1370965158 2011-05-24 Inspection this day found pumps running, well taking water. Crew there this day repairing the concrete bridge from recent flood. Well has good ID's, reading from computer show tubing 1925#, 25# on annulus.
1378427440 2011-11-21 Inspection of this well found everything in order. The gas chart is registering volumes and pressures at well. Proper identification is posted. The pump house is clean and dry. Pressure readings were taken at office, off their computer.
-1408482140 2013-04-30 Unable to check pressure due to lock on building being installed will check pressures the next time I'm in the area, I will contact well operator so they will be on site to unlock building.
-1493669019 2012-02-10 My inspection this day due to quarterly inspection requirements. Pressure readings are taken off company's computer that constantly monitors the wells. The tubing pressure is 1920#, 9# on annulus and the pump is running. The identification is adequate.

1639302194 2009-10-08 Inspection this day due to call from operator - ready to start running packer back in hole - has been rebuilted, going in today. Would like to pressure test Monday (Holiday).
-1684099041 2009-11-20 Inspection this day found location has been reclaimed, grass starting to grow. Nice concrete crossing across the creek, well not taking water this day - still working on getting pump and pump house done. ID's at well will be up this day.
-1713320076 2009-11-20 Well not in use this day - still shut in waiting on pumphouse and electricity to be completed. Should be started up next week, Bruce reports. Reclamation has been completed and looks very nice, especially the concreted creek crossing.
1715421979 2014-08-14 Inspection revealed that injection pump was in operation today and it was noted that creek crossing going to wellhead is in need of repair.
-1719415949 2013-01-16 Inspection on this day revealed that injection pumps were in operation and found pump house clean and neat and facility appears to be in satisfactory condition this inspection.
1905422740 2011-08-02 Inspection this day found no problems, pumps running, pumphouse very clean, good ID's at well, pressures 1920# on tubing, 20# on backside, taken from computer.
1944282670 2014-12-11 Inspection revealed that well was in operation today and it was noted that company has winterized wellhead and associated plumbing coming from buried flow line.
-2033206369 2012-12-27 Restoration work at site satisfactory pit has been filled in and site reclaimed and vegetation has been established.
2046838769 2009-10-13 Tubing, packer pulled as they felt well still had leak - ran packer (witnessed on 10/8/09) and continued running tubing. Ready for corrosion inhib, integrity test this day. Packer reset at 6947.5'. Packer set, valve closed, tubing is un-J'd, 50 bbl fresh water down tubing, followed by 60 bbl corrosion inhib, then followed by 24 bbl. J'd onto packer, pressured up to 2400# on tubing(and climbing as fluid warms up), 2000# on casing - fell 80# over 15 minutes but had noticible leak at well-head that couldn't be tightened with pressure on the well. Will fix leak before injecting. No leak in casing - will pressure test line soon.
-2063610586 2010-11-22 Inspection this day found well house locked, went to facility and read computer system at pumphouse, which shows constant pressure readings on tubing and annulus. Will talk to Tom and see if I need to have them make me a spot or if I can use computer readings - charts sent to Columbus. Talked to T. Tomastik and he said that was fine to use OOG's computer readings for pressure.
-2067919675 2011-02-23 Inspection this day found no violations. Pump running, pressures are 1900# on tubing, 0# on annulus.
-209642850 2013-12-23 Restoration work at site satisfactory pits have been filled in and site restored and vegetation has been established.

-585870517 2012-06-13 Inspection revealed that injection pump was in operation during my inspection.

Note: Curbing needs installed at pump house entrances to prevent any brine from leaving building it appears that when care taker washes out building that there is salt residue out on stone in front of building.

596580621 2009-10-01 Call from Bruce Phipps this morning saying they are going to pull well, starting today, believe it has a small leak. Well passed integrity test for 15 min. but they believe tubing leaking and want to fix it now before they start injecting water. Chief Husted with me this day and we toured oil hauling/purchasing facility, observed new construction on a 35,000 tank and the brine transporting pipeline to the new injection well, as well as visited their other injection wells. Also visited pumphouse and viewed their computerized monitoring system of the oil and brine pipelines in Ohio and WVA. Nice tour by Bruce Phipps.
610607680 2013-12-13 Inspection revealed that injection pump was in operation today and I called company rep Derrick Lamp and discussed getting positive pressure on annulus of tubing on this well and the Long Run facility.
612281087 2009-09-24 This day finished running packer on tubing in well. Ran Baker Lok-set packer with down hole shut off valve and on/off tool. Set packer at 6981' G.L. and will let set overnight. Previous day witnessed packer going in hole.
633289195 2009-08-20 Inspection this day to see if rig finished drilling. Found pipe out of hole, have lost cone on bit and trying to get back on bottom. After pulling bit, found cone missing so went back in with magnet to recover cone, could not get back down last 9 joints of hole - hole will have to be reamed for that distance with rerun bit and going back in now with it. Cone may have been drilled up or if still in hole, bit will have to come out again and go back in with magnet.
-747067655 2013-08-21 Inspection revealed that injection pump was in operation during my inspection.
-993769372 2010-01-26 Tested tubing overnight. Left 2000# on tubing overnight, next morning was 1700#. Service rig on location. OOG worried about lose, Tom Tomastik suggests just wait until it fails integrity. This day pressured up tubing back up to 2000#, held there for 15 min. with only 11# drop. Will continue to use.
