-1089047662 2013-06-24 I was on location at 7:30am to witness the plugging of this well. Petroset set plugs across the Big Lime, at the bottom of the surface casing, and from 200' to the top of the hole. Thompson Well Service recovered 75 joints of 4.5 casing. The hole circulated on each plug and the cement held in the casing after the job. This well has been properly plugged. See plug report for details on this job. No violations. Phone Call
1112198397 2005-03-23 CASE 05-002 The Uniontown office received a call from Chief Don Harris with the Garrettsville Fire Department reporting an oil spill near 9320 SR 88. The well was identified as the Schneeberger #1. According to Dennis Kingsberry, the contract pumper, the flowline became blocked with salt and the back pressure pushed oil and gas back through the stuffing box. The release was estimated at 10 to 15 bbls. The release flowed 150 feet east toward a small unnamed stream. There was no evidence of much oil having reached the stream. The majority of the spill was captured or slowed by vegetation. Northeast Oilfield Service provided a vacuum truck and Waldron employees provided booms, pads, peatsorb and manpower to contain and remediate the spill. Since the spill impacted surface water, the OEPA-DERR was notified. Northeast recovered approximately 25 bbl of oil and water. The impacted soil and vegetation was placed on plastic and bermed with straw bales. The material will be disposed off site.
1146332803 2013-11-06 I was on location to check the final restoration status of this plugged and abandoned well. Final restoration is due by 12/24/2013. The lease road is still being utilized for access to the Schneeberger #2 (PN# 22241). The location has been graded but has not been seeded. The power pole that was used for the pumping unit is lying on the ground on the north side of the lease road. I called the contract pumper to inquire about the status of the location. He told me that he would be removing the power pole in the next week and that the landowner has been stockpiling topsoil in the former location of the well to raise the ground level. All equipment has been removed from this location. Final restoration is pending the removal of the pole. No violations.

-1191514065 2005-07-12 CASE 05-002 The impacted soil has been disposed and the impacted area has been seeded and mulched or has revegetated. The violation has been corrected.
1375987061 2013-06-21 I was on location to witness the plugging of this well. Waldron, Petroset, and Thompson Well Service were on location to plug the well. The bottom plug was set through 2.375 tubing set at a depth of 4464'. Petroset rigged up, broke circulation, and mixed and pumped 30 sacks of Class A cement. The cement was displaced with 15 barrels of water to a depth of 4072'. The trailer was spotted and Thompson began pulling tubing. Mechanical breakdowns and issues with the wellhead and pits delayed the job and it was decided to pack the wellhead off and finish the job on Monday. No violations. -1661268516 2005-04-14 Met on site with Dennis Kingsbury, the contract pumper and discussed what would be necessary to finish the spill clean-up. Actions included removing the soil stockpile, removing any remaining impacted soil or vegetation, grading and seeding.
-1900361135 2005-06-22 The soil stockpile has been removed. Waiting for the vegetation to establish before closing the violation.
-2055327643 2013-06-20 I was on location to check the status of this well prior to abandonment. Ted Cutright (D.A. Waldron) and Thompson Well Service were on location. The rods were successfully pulled from the tubing and did not require steaming. Petroset is scheduled to be on location at 7:30am tomorrow to set the plugs. No violations.

973814348 2013-10-09 I was on location to check the final restoration status of this well that was plugged on 6/24/13. The code I had for the electric gate was no longer valid and I was unable to access the location. I called Ted Cutright (Waldron) and asked him for the contract pumper's phone number so I could contact him and set up a time to meet on location. I got the phone number and will try and meet him next week for inspection. No violations.
