-401234289 2015-01-20 I inspected this well as a follow up to NOV # 1904643198. As the previous 19 inspection reports indicate this well has not meet the prescribed remedial requirements as detailed previously. The well head consists of 8.625 X 4.5 casings with 1.5 tubing. The well head is incomplete and not able to produce. There is a breach in the 8.625 casing head and gas and oil could be seen bubbling from it. Previous conversations with Brian Carr ( Maverick Oil & Gas ) indicated that the 4.5 casing has lost its integrity and is leaking oil and gas into the annular space and up into the 8.625 casing. It was not possible to determine the area affected by the contamination due to fresh snowfall. The tank battery is shared with the Fabro # 1 ( 34-153-2-0791, Atlas Energy ) and is comprised of a 100 barrel oil tank, 25 barrel steel brine tank, 2408 horizontal mechanical separator with orifice gas meter. There was not a chart on the gas meter. The tank battery is fenced and locked. The requirements set forth previously have not been complied with.
796307146 2013-04-27 I inspected this production property on 4/27/2013. The condition and status of this well and associated equipment has not been rectified since last ispected by Robert Worstall in 2011. The tank battery consists of a !00 Bbl. Stock tank, 25 Bbl. Brine tank, 2408 Gas separator with Gas measurement meter. Conversations with Brian Carr indicated that this well could possibly have a hole in the 4.5 casing, which would require a repair. The Flow line needs replaced according to Mr. Carr. Upon my inspection of the well head itself, I discovered a oil release that was around the 8.625 casing head. The size of the oil release 10' X 8' . The oil appeared to be oxidized and not recent. Pieces of the wellhead plumbing were missing and could be a source. This well has not reported production since 1995 and is incapable of production. No change since last inspection.
