1209075449 2008-04-24 At 8:00AM I received a call from Marlene Hall about a complaint. I proceeded to the location and arrived on the scene at 8:20AM. The EPA, Kurt Kollar and clean up contractor, Sweet Meadows, were talking about the way to attack the spill. The Duck Creek pumper, Jeremy, was on the scene too. Jim Oberle, the owner, operator arrived on the scene at 11:00AM. Also on the scene were the Jackson Township Fire Department chief and Milton Township School ground supervisor, Jim Roberts and the school principal. The pond and the marsh area west of the pond were on the school's property. The spill meandered through the creek, covering a 1/2 mile span. A siphon dike was installed approximately 100' from the tank battery. The span from the tank battery to the siphon dike was marsh land. Two soft booms were placed on the east side of Duck Creek road outside the siphon dike. The creek went under Duck Creek road, west to a small pond and another marsh area. A hard boom was placed at the west end of the pond with a hard and a soft boom at the east end of the pond. Six more soft booms were placed throughout the marsh area west of the pond. Sweet Meadows proceeded to burn off all the oil that had gone over a beaver dam on the west end of the pond. Small pockets of oil, west of the pond, were burned Sweet Meadows will start again tomorrow around 7:00AM.. Rust from the heater tubing on the tank battery caused the failure. The bottom of the 110 barrel tank had rusted through.
1209314836 2008-04-25 I performed a status check on the oil spill. EPA, Kurt Kullar was on site as well as owner, operator Jim Oberle. Sweet Meadows were flushing water through the marsh to get oil down in one area. The tank battery area will be cleaned up on 4/28/08. There were no further violations at this time.
1209915740 2008-05-01 I met with Jim Oberle, owner/operator and EPA rep, Kurt Kurhler and Sweet Meadows, Rodger and Ben at the tank battery site. I witnessed the digging up of the contaminated dirt and removing to a land fill. Sweet Meadows recovered 16 tons of dirt, rock from the dike area. Sweet Meadows brought in dirt to build the tank battery area up. They dug a trench around the outside of the dike area. I found no more violations at this time.
1212693436 2008-06-06 I performed a NOV follow-up. The ID sign was posted at the tank battery. A Danger sign was also posted. Rock was layed inside the dike area. This case is resolved.
-1421962860 2008-05-13 I performed an inspection of the tank battery area. I calle Mr. Oberle and he stated that they were still waiting for dryer weather. I agreed. Case Continues.
1604928501 2008-05-12 I performed a NOV follow-up. The dike area still needs the rock. It had rained yesterday and today. I called Mr. Jim Oberle and he stated that they are waiting until it dries up so the rock wouldn't sink down in the mud. I will continue to monitor. I found no more violations at this time.
-1736314938 2008-04-24 There was a tank battery spill. The cause of the spill was the bottom of the tank battery had rusted through. Owner/Operator, Jim Oberle(330-345-8796) was on site. I talked to him about the spill and the tank. I let Mr. Oberle know that the tank needed to be replaced and the dirt and the dike needed to be removed, hauled to a dump site and new dirt hauled in .
-2031908574 2013-03-12 On 3/12/2013, I performed a status check on the above well site. The tank battery is equipped with one 100 barrel tank and one 50 barrel tank and one horizontal separator. This is a 1.5 plunger lift well . Identification signs are posted. No violations are noted at this time.
434331660 2008-04-29 I met with EPA rep. Kurt Kullar and Sweet Meadows reps. Rodger and Ben. We all discussed the status of the oil spill. The pond is around 95% clean. The upper ditch area is 95% clean. The marsh area west of the tank battery and the tank battery dike still need work done. The dike area will be dug up on 5/1/08, weather permitting. Case continues.
457680663 2008-05-07 Inspected the tank battery area. There was no rock hauled yet. I called Sweet Meadow's, Rodger Greenwalt. He said his truck that has the rock in it is down due to brakes have gone out. I told him to call me when the rock gets hauled to the battery area. There were no violations at this time.
829433223 2008-06-04 I performed a status check on the tank battery. The dike was up and the rock was installed inside the dike. The northwest corner of the dike still had a small amount of oil leaking on the outside. Sweet Meadows contractor dug up that area of dirt and replaced it with good dirt. The tank battery is almost done execpt for the ID sign. I called Mr. Oblere and was told that the ID sign would be installed by the end of the week. This case continues.
98791416 2008-04-28 I met with Jim Oberle, Sweet Meadows, Rodger and Ben about the status of work to do. The pond to the west was almost cleaned up. West of pond is all clean. Working from pond east to the tank battery. Tank has been removed from the dike. Waiting for weather to clear up so they can dig oily dirt out of dike area. Case continues. There were no violations at this time other than the NOV.
