1339591921 2012-06-12 On 6/12/2012, I investigated a gas leak reported by Captian Maffit of the Champion Fire Department. My inspection found a well belonging to Dustin Energy Inc. was leaking natural gas from where the tubing and 4.5 packing ring meet and the identification sign on the separator was faded and unreadable. I called Bill Templeton at 330-240-5573 of Dustin Energy Inc. and informed him the well head needed repaired and a new identification sign needed posted. I will conduct a follow-up inspection by 06/25/2012.
1344948415 2012-06-26 On 6/26/2012, I conducted a follow-up inspection for a plunger lift unit leak. My inspection found the well still leaking between the tubing and 4.5 packing nut. I called Bill Templeton at 330-240-5573 and asked to have this leak fixed. Inspection not passed.
1346350881 2012-08-27 I met with Bill Templeton of Dustin Energy, Inc and conducted a follow-up inspection. My inspection found the leak at the well had been repaired and a new identification sign was placed at the tank battery. Inspection passed.

-913677420 2012-08-14 On 8/14/2012, I investigated a complaint at 8:45am that was received on 8/13/2012 about a gas leak at the well head. My inspection found the well was still leaking natural gas and the well was shut in. I had called Bill Templeton at 330-240-5573 on 8/13/2012 to have his well tender fix the leak. Bill called me at 1:30pm on 8/13/2012 to inform me that he and well tender had gone out, tightened the 4.5 packing nut, and stopped the leak. I called Bill this morning to inform him that the well was leaking again and told him this was the second compliant I received within the last two months. Bill said the well tender had been dispatched to purchase new packing to repair the leak and will call me when the repair is complete.
