1271342610 2010-04-13 Open pit located 12 feet from the well. Size 15' by 6' by 6'. Pit must be closed. Well site must have vegetative cover. Seed and mulch are recommended to establish vegetative cover. Access road is eroding and needs to be graded and vegetative cover established to prevent erosion. Diversion ditches and water bars are recommended to control surface run off and to prevent erosion. Well must have identification: Well owner, lease name, well number, permit number, county, emergency telephone number. Well is equipped with 1.5 tubing. Well is not connected to production equipment or production lines. No equipment on location. Well needs to be capable of production or plugged. Restoration is past due and must be done following Best Management Practices.
-1902361092 2010-09-29 Pit closed, site graded, vegetative cover established. Well is equipped with tubing, Connected to production line. Access road graded with good vegetative cover established. Restoration passed.
-933591474 2009-03-05 No identification. Pit is open and full of water. Water is seeping through the unlined ditch between the wellhead and the pit. Pit has overflowed the liner. Pit needs to be pumped out and closed. Wellhead has 4.5 inch casing with a valve that is closed. Both 2 inch annular valves are closed. I called Dave Strang and he will take care of pumping out the pit and closing it. They are in process of fracing other new wells in area and will frac this well and then reclaim on their way out of location. Pump water out of pit and dispose of in an injection well. Close pit and reclaim location seed and mulch exposed areas. Grade site to achieve positive drainage.
