2040833 2012-02-02 No Violations Noted First Class Energy was contracted by Penn View (Wayne Brannon ¿ tool pusher). Well sign and E&S plan were located at the access road. No freeboard issues were present at the site. Conductor pipe (13 3/8¿) was set at 42¿ (sanded in place). First Class drilled to 200 ft. and noted high water bearing zones at 80 ft., 120 ft. and 160 feet. High water bearing zones during drilling caused a semi- loss of flow of drilling cuttings and water through the diverter pipe to allow water/drill cuttings to flow on back side of conductor pipe causing a release around the rig. First Class stopped drilling and cemented the backside of the conductor pipe with 5.2 sks of Class A. Plug was down at 4:00 pm on 2/2/12 with no return. First Class was going to allow the 8 hrs. Cement time for set up of cement prior to drilling to 300 ft. to set a water protection casing. April Wieland and March Maestra were on-site and noted the release and other site issues.

First Class Energy was actively drilling during inspection. Access road had little to no stone. Mud was being dispersed from the site onto the township road. Pit was poorly constructed. Liner was punctured with metal posts holding safety fence. Liner was not smoothly or evenly distributed throughout the pit. Rig was spewing cuttings and mud onto the ground. Rig hand said they hit a "wet spot." Wade Branan (tool pusher) stated they hit three separate water bearing zones. E&S controls was poorly installed and not maintained. Sediment laiden runoff was making its way past the silt fence. Based on E&S plan cuttings cannot be habdled on site due to the seasonal high ground water table. PA DEP Water Quality Specialist April Weiland was present during inspection.

No freeboard issues were present at the site. Water Protection pipe (9 5/8¿) was set at 295¿. Due to the potential of loss of circulation, cement basket was set at 42 feet. Pipe was cemented with 120 sacks of Class A with 8 bbls of return. Plug was down at 6:44 am on 2/3/12. Surface pipe (7¿) was set at 1,053 feet with 190 sacks of Class A with 5 bbls return. First Class Energy was currently drilling at 1, 570 feet. The anticipated total depth for the well was 4,640 feet.
2041530 2012-02-09 No Violations Noted First Class Energy was contracted by Penn View (Wayne Brannon ¿ tool pusher). E&S controls and well sign were present. No freeboard or E&S issues were present at the time of inspection. Well drilling was completed. First Class was removed from the site.
2043970 2012-02-16 No Violations Noted Penn View had contracted Appalachian to frac well. E&S controls and well sign were present. No freeboard or E&S issues were present at the time of inspection.
2048860 2012-03-06 No Violations Noted Site visit was compliance evaluation from 2-2-12 violation.

Well drilling was complete and well was being frac'ed. Jim Baerer was on site (construction). Well cuttings had been sampled by Waste Management for landfill disposal. Site was being rocked for water truck access to pit for water removal. Silt fence was repaired. Entrance to location was rerocked. No further violations noted at this time. PA DEP Water Quality Specialist April Weiland was present during inspection.

2131671 2012-12-11 No Violations Noted At time of inspection, well equipped for production. On-site brine tank appeared sound. Access road in good condition. Well site had required 70% vegetative growth.

No E&S issues noted, although Department recommends that Operator maintains BMPs as structural integrity appears to be compromised which could lead to a potential pollution event.

Unable to locate restoration report for this site. Spoke to Operator Brad Little via phone on 1/18/13 who agreed to re-submit report.

UPDATE: 3/08/2013 Restoration report received by Department.
