Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION_COMMENT
2132721 2013-01-22 No Violations Noted At the time of inspection the Operator was in the process of cutting and removing trees and underbrush. The inspection at the request of the Operator to inspect perimeter BMP controls prior earth disturbance. At the time of inspection the Erosion and Sedimentation plan was on site and implemented as designed. No observation of accelerated E&S issue in relation to proposed well pad area or the access road. The Department suggests that the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2139208 2013-02-13 No Violations Noted No Violations noted
2152936 2013-04-02 No Violations Noted At the time of inspection the Operator was in the process of constructing the site.
2163800 2013-05-02 No Violations Noted At the time of inspection the Operator was in the process of constructing the site.
2174950 2013-06-13 In Compliance with Policy Inspection the result of the Operator reporting a breached BMP during a precipitation event. The Operator is in compliance with the Departments notification policy for a BMP breach. During the inspection the Department notes that an infiltration ditch located closest to the access road failed during a heavy precipitation event in the early morning hours on 6/13/2013. The failure resulted in material from the infiltration ditch being washed into a 32" silt sox, which did not contain the material and also failed. The two failures resulted is accelerated erosion of the access road and scouring of a hillside below the excavated pad area. The precipitation event appears to exceed the Departments criteria for construction standards. The Operators response was to rebuild the failed portion of the infiltration ditch, add an additional rock outlet and repair the silt sox. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2175884 2013-06-13 No Violations Noted Conductor being cemented in at time of inspection. Inspection conducted w/OGI J. Branan.

2177611 2013-05-24 No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Department notes that the Operator replaced the silt sox, installed an additional rock outlet, and repaired the failed earthen dike surrounding the well site. The Department has also notes that the additions have been added to the E&S plan. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2182013 2013-07-05 No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. No observation of accelerated erosion or sedimentation in relation to the access road or well site. The E&S plan was on site and all associated BMPs appear to be installed as outlined on the plan. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2182377 2013-07-09 No Violations Noted Waiting on rig to commence tophole drilling.
2185968 2013-07-24 No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Operator indicated that they were actively drilling the well and also indicated they were at approximately 1900' in depth. During the inspection the Department observed two areas of concern in relation the well pad and access road. The first location is the outlet of the perimeter ditch on the East side of the well pad. The Department observed sedimentation in the outlet rip rap on both sides of the road. The second area is located within the LOD between the well pad and area reserved for the CI. The Department suggest the operator add clean durable rock to the make shift basin and outlet up to the access road, and also clean the reinstall the silt sox to prevent sedimentation from leaving the LOD. The Department also notes that hay bales are not ABACT approved in HQ watersheds. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2188818 2013-08-01 No Violations Noted The issues outlined on the 7/24/2013 inspection # 2185968 have been addressed. During the inspection he Department observed both areas of concern and the temporary stabilization techniques the Operator employed to address the concerns of the Departments. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2189897 2013-08-06 No Violations Noted At the time of inspection the drill rig was located onsite and placed on containment. The Operator indicated that the well was drilled to the KOP. See inspection #2189814 for description of observations made during site inspection. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2195024 2013-08-07 No Violations Noted Waiting on rig to commence lateral drilling.
2209074 2013-10-02 No Violations Noted At the time of inspection the well was drilled to KOP. The E&S plan was onsite and all BMPs appeared be installed and maintained as outlined in the plan. No observation of accelerated erosion or sedimentation in relation to the well pad or access road. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
