| 631135038 | 2014-10-25 | At 2:26AM I received phone call from Bruce Carpenter of Eclipse about a flash fire at the Pora Pad during flowback with one individual being injured in the incident. Upon arrival on the day of my inspection I met with Eclipse representatives Bruce Carpenter, Jim Chambers and Mike Boswell. Ameriflow had been on site flowing back the 6H when Excel employee Tommy Miller noticed gas at the flowback tank and went to shut down the pump manifold as he was shutting it down the pump ignited the gas at the flow back tank starting a flash fire which consumed the flowback tank, a roll off box, secondary containment and two canopy |
