1857560 2009-01-21 Violation(s) Noted The Department conducted an investigation in response to a spud report that was sent in. Upon inspection of the E&S controls the Department noticed dark green staining of the snow that was left on site. After closer inspection of the unknown discharge, the Department spoke with two people on site to ask if they knew what had happened. The drilling company stated that the night crew had blown a hydraulic line and they didn﾿t know that it had reached the controls. I immediately telephoned Mark Hopkins and Mike Cioffi and let him know of the discharge. (CONT.) 579825 91.33A - Failure to notify DEP of pollution incident. No phone call made forthwith
1858242 2010-01-25 No Violations Noted The Department conducted a compliance evaluation inspection for the violations noted on 1/21/10. (Operator discharged industrial waste to the ground.)(Operator failed to notify the Department of a pollution event.) On the date of the compliance evaluation inspection all of the above referenced violations have been abated. The Department also noted that during the inspection of the E&S controls, they began to be over topped from collection of water at the site. The Department contacted Mike Cioffi and he stated it would be taken care of immediately.
