Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
1867295 2009-02-18 No Violations Noted drilling @ 1470ft/E&S ok/ID&pit ok
1865395 2010-02-24 No Violations Noted Drilling was completed at the time of the inspection. Below grade pit located onsite and the pit had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation with respect to the access road. Observed evidence that runoff had transported a limited amount of sediment off-site during the most recent rain and snow melt. The operator had restablished the filter fabric fence after observing the compormised filter fabric fence. The operator should closely monitor the site for sediment transport during future precipitation events and/or snow melt due to the proximity of the site to an unamed tributary to the Allecheny River. Spoke with operator representative Mark Hopkins concerning the site and possible erosion issues.
1884418 2010-05-14 No Violations Noted Well appeared to be equipped for & in production at the time of the inspection. Brine tank located onsite and appeared to be in sound condition. Site had been regraded, seeded & mulched and the vegetation had begun to grow. Vegetation did not as of yet meet the 70% uniform coverage criteria. No accelerated erosion & sedimentation was observed in relation to the site or access road. The operator should continue to monitor the site to insure the erosion control BMP's continue to function properly until the vegetative cover achieves the 70% uniform coverage criteria.
